Job Description

DivisionChronic Disease & Injury PreventionTemporary DurationApproximate Duration: 9 to 12 monthsJob DescriptionJOB SUMMARYUnder the direction of the Manager, Chronic Disease and Injury Prevention, the Community Health Broker (CHB) makes an impact on health and health equity by engaging with identified vulnerable populations to connect them with each other, local resources, and community assets that empowers living healthier. This role will work to gain support and credibility with individuals, families, communities, and neighborhoods and will address health issues by educating, advocating and promoting and/or collaborating with various health services/supports throughout Niagara. The CHB will be guided by Public Health’s strategic plan and priorities, the Ontario Public Health Standards, and best evidence available. This role will be responsible to develop and maintain internal and external partnerships for the purpose of health promotion work at the community and neighbourhood level. Work will be conducted within a multi-disciplinary team including health promoters, youth engagement specialists, public health nurses, dieticians, program assistants, and various external partners to reduce health inequities utilizing a neighborhood, community-based outreach model.QUALIFICATIONSEDUCATION

  • Bachelor’s degree in community development, human service field, or equivalent knowledge from experience working with vulnerable populations using anti-oppression and empowerment frameworks is required
  • Current CPR/First Aid Certificate is an asset.
KNOWLEDGE/EXPERIENCE
  • 4 years’ compressed experience working directly with vulnerable populations in a similar community development/engagement role is required.
  • Experience in planning and implementing community-based and health-minded programming are required.
  • Knowledge of working from anti-racism, anti-oppression and harm reduction frameworks are required.
  • Demonstrated knowledge of the effect of health inequities/inequalities and its impact on residents and their families, which includes the effects of poverty, marginalization, addiction, mental illness and isolation are required.
  • Demonstrated knowledge and experience of working with vulnerable populations as individuals and as communities are required.
  • Knowledge and experience working with residents across the life course with mental, physical and cognitive challenges are an asset.
  • Demonstrated ability to provide mediation for residents in crisis and a strong understanding of other community services and resources to enable appropriate interventions are required.
  • Experienced in note taking, making referrals and providing relevant follow-up with community organizations and clients are required.
  • Experience in partnership work, community collaborations and collective impact are an asset.
SKILLS
  • Able to initiate connections and actively engage with vulnerable populations who have challenges with multi-sector complex needs through a variety of methods is required.
  • Strong oral communication and interpersonal skills are required.
  • Able to assist individuals/communities in developing core personal skills: personal care, health and wellness, communications, safety and problem solving is required.
  • Demonstrated ability to engage residents, gather feedback and plan initiatives based on resident needs are required.
  • Demonstrated aptitude for creativity and out-of-the-box thinking, with a willingness to explore new and unique ways of service delivery is an asset.
  • Willingness to take initiative, and the ability to assess and initiate actions independently to meet the needs of the residents is an asset.
  • Strong group facilitation skills is required.
  • Excellent organization and time management skills are required.
  • Strong ability to work independently in community and plan in a multi-disciplinary and collaborative team with the ability to work in various environments (e.g. office and community) are required.
  • Adept at dealing with sudden and significant events that could have a negative impact is required.
SPECIAL CONDITIONS
  • Must possess and maintain a valid and unrestricted specified license for the class of the vehicle operated.
  • Must maintain ability to travel in a timely manner to other offices, work locations or sites as authorized by the Corporation for business reasons.
  • In accordance with the Corporate Criminal Record Check Policy, the position requires the incumbent to undergo a Criminal Records Check / vulnerable sectors screening and submit a Canadian Police Clearance Certificate.
  • Work flexible hours as required in response to community based health issues. Such hours of work are scheduled in accordance with the collective agreement.
