Job Description

DivisionHomelessness Services and Community EngagementTemporary DurationApproximate Duration: 6 - 12 monthsJob DescriptionJOB SUMMARYReporting to the Manager, Homelessness Services, the Coordinated Access Program Analyst will support the Built for Zero Canada initiative, which is a national change to support communities end chronic homelessness. This role is responsible for the implementation and on-going work of the By-Names list and coordinated access program in collaboration with the partnering agencies.EDUCATION

  • Post-Secondary Degree in Social Sciences, Human Services, Business Administration or related degree
KNOWLEDGE
  • At-least 3 years’ experience working with low income or marginalized clients
  • At least 3 years’ experience in a human services environment working with multiple social/community agencies.
  • At least 3 years’ experience working with community agencies and collaborators.
  • Working knowledge of By-Name Prioritized list, coordinated Access and SPDAT Assessment Tool an asset
  • Crisis Intervention Training.
  • Knowledge of homeless coordinated access programs, community services delivery system and social policy issues, databases and statistical analysis software.
  • Knowledge of computer logic and applications.
RESPONSIBILITIES1. This position provides on-going support to community partners and staff
  • Supports and facilitates the homelessness coordinated entry system
  • Participates and facilitates regular community meetings with community partners.
  • Assists in the planning and delivery of the homelessness enumeration mandate in partnership with community agencies.
  • In partnership with homeless team and agencies, develop Niagara’s priority populations for coordinated access.
  • Work with all housing providers and agencies in Niagara to encourage participation in coordinated access program.
  • Establish acuity thresholds and map to housing resources within the coordinated access system.
  • Works closely with team members and community partners to identify systemic barriers within the housing and homelessness system, and develop strategies to overcome homelessness.
  • Documents and works to remove barriers related to emergency shelter, prevention and diversion resources/approaches identified through the coordinated entry system.
  • Develops and offers regular training for community partners and stakeholders.
  • Monitors and assesses community homelessness plans in accordance with Federal, Provincial and Regional legislations and community needs.
2. Facilitates the assessment of clients in Homeless shelters
  • May conduct V-SPDAT and SPDAT assessment on clients in shelters greater than 21 days or as required.
  • Identifies priorities and adheres to all organizational policies and procedures and makes recommendations where applicable.
  • Plays an integral role in the data sharing process.
  • Works within an evidence-based, Housing First philosophy.
  • Ability to manage complex and unexpected situations with frequent changes, delays and multiple requests from many partners.
3. Stakeholder Engagement
  • Helping the Niagara Region community adopt proven practices, deploying existing resources more efficiently, and using real-time data, rapid cycle testing and human-centered design to improve performance.
  • Engaging leadership from the government, private and philanthropic sectors in securing new resources for communities and removing policy roadblocks.
  • Connecting agencies to one another for innovation, knowledge capture and group problem solving.
  • Develops, maintains and supports professional relationships within the division, department and with community partners/agencies, sharing information, ideas and other communications across service agencies for consistent messaging and training
  • Supports the work of the 10 Year Housing and Homelessness Action Plan.
4. Works with colleagues and service providers to identify and pool available data resources to reduce costs and redundancies.
  • Acts as community resource with focus of disseminating available data to community organizations/ working groups as requested to assist community with data driven planning.
  • Supports translation of data for communities to assist with planning, developing strategies to move data from report format to useful community capacity building tools.
  • Acts as a primary liaison for community partners to make effective and efficient use of information systems in order to increase their ability to provide relevant and accurate data in accordance with the principles and requirements of the Federal, Provincial and Regional government.
SPECIAL REQUIREMENTS
  • Incumbents must maintain the ability to travel to various locations in the Region for meetings and other business reasons.
  • In accordance with the Corporate Criminal Record Check Policy, the position requires the incumbent to undergo a Criminal Records Check and submit a Canadian Police Clearance Certificate.
