Job Description

Temporary DurationApproximate Duration: 6 - 18 monthsJob DescriptionJOB SUMMARYAs part of the HNHB LHIN Falls Prevention Strategy and under the direction of the Wellness Services Manager, incumbents will plan, implement and evaluate a community based exercise program and ensures alignment with the provincial requirements. Keen observation skills and individual participant assessment will determine modification/adaptation needs within the group on a regular basis. Provide functional direction, co-ordination, and implementation of community based group exercises for seniors at risk for falls, work collaboratively with other stakeholder groups, and promote health and wellness activities to a senior population in a group setting. Collect statistical data, develop and administer surveys and prepare and submit monthly reports to the Wellness Services Manager.EDUCATION

  • College diploma in fitness and health promotion or equivalent
  • Certified Fitness Instructor (Canadian Society for Exercise Physiology) (CESP-CPT) preferred or Canfit Pro with an Active Ageing Certification (Certifications require annual registration to maintain the certification and maintaining a valid CPR/First Aid Certificate)
  • College Certificate in Gerontology preferred
  • Additional training in SAMS Steps exercise preferred
KNOWLEDGE
  • 1-2 years in community-based program development, program planning, implementation and evaluation with an emphasis on community mobilization
  • 2 – 3 years demonstrated experience leading community fitness programs with specific training in offering exercises to seniors
  • 1-2 Years working with seniors in a community based program
  • Sound knowledge of community support services for seniors within Niagara
  • Demonstrated knowledge of principles of falls prevention and the continuum of exercise support (Stand Up model, SMART model and, SAM STEPS models of exercise) through the strategy
  • Ability to provide modifications based on participant need
  • Proven ability to work within a multi-agency team and leverage community resources to support program goals
  • Experience with computer applications (Database system i.e. CRMS, Access, Excel, Word, PowerPoint and internet applications)
  • Knowledge of client/participant safety issues and demonstrated capacity to apply client/participant safety practices

  • Excellent communication and interpersonal skills
  • Demonstrated ability to perform community needs assessment; program planning, design, implementation & evaluation; group facilitation, advocacy
  • Demonstrated leadership, organizational, and community engagement skills.
  • Strong initiative and proven ability to work independently as well as cooperatively within a multi agency framework to achieve identified target outcomes.
SPECIAL REQUIREMENTS
  • Good attendance record.
  • Valid drivers licence, reliable transportation, and ability to travel throughout Niagara to various locations.
  • Available to work evenings and weekends as required.
  • In accordance with the Corporate Criminal Record Check Policy, the position requires the incumbent to undergo a Criminal Records Check, including a vulnerable sector search, and submit a Canadian Police Clearance Certificate.
Shift InformationCurrently scheduled 16 hours per week various shifts, may be required to work days, evenings and nights including weekends.
