Is mindful of the psychological health and safety of our employee care partners.Qualifications:· Grade 12 Diploma or recognized equivalent is required.· Graduate from a recognized Health Care Aide program. Remains active as a certified Health Care Aide.· Valid Certification in First Aid and CPR not required but an asset.Responsibilities:As a member of our Care Partner Team, your role will include: · Provides care in a manner that demonstrates the principles of appropriate choice, dignity, privacy, independence, and individuality in accordance with the Mission, Vision, and Core Values.· Collaborating with the Community Care Partner team to create a Community where people are safe, respected, and comfortable.· Helping people become well-known in the Community.· Understanding the Domains of Well-Being and bringing them to life daily.· Keeping the Residents’ preferences and needs at the center of all your decisions, conversations and actions.· Supporting the rhythm of daily life under the guidance of the Clinical Mentor and in accordance with the preferences of the Residents, our Well-Being Strategies, and applicable regulations.· Actively participating in Community Learning Circles, meetings and events.· Ensuring that the voices and preferences of the Residents are accurately captured in their care/service plans.· Being a good care partner to others in the Community.· Taking on new skills that enable the Residents to engage in their normal life patterns and make our Community a pleasant place to live and provide services.· Supports the Resident’s lifestyle in a community environment in accordance with the Resident’s individualized Care Plan as authorized by the Homecare Nurse.· Supports and participates in Resident centered activity program. Assists the Resident with appropriate leisure/recreational activities daily as per the Resident’s previous lifestyle, functional skills and emotional needs.· Assists the Resident with daily light housekeeping duties in the Resident suite and performs light housekeeping throughout the community.· Assists Residents in the Dining Room as necessary at mealtime.· Assists the Resident with laundry as directed.· Assists the Resident with personal care as authorized by the Homecare Nurse.· Provides medication assistance to the Residents, according to current Well-Being Strategies.· Attends and participates at meetings as requested as part of the care team. Participates in the development of Resident Care Plans. Maintains records as necessary.· Demonstrates an awareness/knowledge of the importance of health and safety; accident and injury prevention; and adheres to safe work practices and procedures.· Identifies any health, safety or maintenance issues, and reports maintenance issues as required.· Follows and enforces the Emergency Management Plan, Infection Control protocol, WHMIS, and Health & Safety Wellness Strategies.· Identifies and reports in a timely manner any changes in resident health status to the LPN/Supervisor.· Exhibits excellent hospitality and customer service relations by consistently showing courtesy, caring, understanding, compassion and respect to all Care Partners.· Maintains a high level of professionalism at all times.· Participates in ongoing educational programs.· Complete general housekeeping duties as per Wellness Strategies.· Maintain confidentiality of the Community including but not limited to accountability of all care partners. Breach of confidentiality is serious and may result in disciplinary action up to and including termination.· Performs other services/tasks/duties related to employee care partner role as assigned.Employment is conditional upon providing satisfactory proof that a candidate is fully immunized against COVID-19.
