Job Description

Our organization is seeking Treatment Coordinators who are passionate about preventing the spread of HIV and other diseases in our community. The Treatment Coordinator will be responsible for updating information about new treatments, communicating with patients regarding new treatment options, and monitoring the effectiveness of existing treatments to help prevent the spread of disease.Responsibilities:

  • Assist in the implementation of professional treatment plans.
  • Provide home-based treatment interventions to ensure that clients adhere to treatment plans and that they receive the highest quality care possible.
  • Monitor client progress and ensure proper home care.
  • Document all interactions with clients; facilitate outreach and family education at the request of the treatment team.
  • Plan and implement treatments, interventions, and programs.
  • Establish client care plans; assist in the development of specialized treatment plans based upon a variety of factors (i.e., age, gender, culture, and ethnicity).
  • Facilitate client follow up and evaluation; communicate progress to referring physicians.
