Salary Band $51,719 to $61,920 per yearWe are currently seeking a permanent full-time Community Support Worker - Release from Custody (Addictions focus) position to work with clients of the Mental Health & Justice Services Program and provide discharge planning services to persons with a serious mental illness who are in custody. The worker will assist in the reintegration/integration of recently released offenders into their home community or community of choice.What you will do
  • To assist individuals with mental health concerns who are being released from custody establish release plans.
  • To liaise with respective counsel and crown attorney supporting the release from custody
  • To engage with and provide a needs assessment of offenders with mental health and addiction concerns.
  • Once supported with a release from custody plan, to provide community case management for individuals to support their mental health and reintegrate successfully into the community or support in connecting to appropriate services if they will be residing out of the Peel Region
  • In collaboration with the client, their significant others and the community partners create a plan for addiction treatment, considering the social determinants of health
  • Facilitate referrals to residential, day, mutual aid, self-help and/or out-patient/community addiction services as well as other supportive services as required
  • To provide education to the community agencies and institutions regarding mental health discharge planning services
  • To communicate the release plan to service providers in the clients’ community
  • To conduct case conferences/consultation with treatment providers, including physicians, other community organizations and other significant persons, as deemed appropriate.
  • To refer and link clients to appropriate services and respectfully advocate on their behalf when appropriate.
  • To attend regular case conferences at the institutions as required.
  • To establish and maintain working relationships with Institutional Social Workers, Probation Officers, Consulting Psychiatrists, Institutional Personnel, community agencies and schedule 1 hospitals.
  • To provide education to the community agencies and institutions regarding mental health discharge planning services.
  • To participate in the agencys Addiction Community of Practice, provide group counselling and education groups as part of the agencys Addiction Community of Practice and provide subject matter expertise as part of the MHJS team
  • In collaboration with institutional staff, develop relevant policies and procedures for the discharge planning services.
What you need to join our team
  • Bachelor of Social Work or related Social Service undergraduate degree required.
  • Registration with Canadian Addiction Counsellors Certification Federation an asset.
  • Minimum of 3-5 years of demonstrated experience working in the addictions and/or mental health field.
  • Demonstrated cultural competence and the ability to support a diverse client population are essential.
  • Working knowledge and understanding of the Criminal Justice System and Recovery Model.
  • Excellent oral and written communication, organizational and problem solving skills.
  • Comfortable working in a community setting and institutional setting
  • Demonstrated ability to work independently and as part of a team.
  • Proficient computer skills including MS word, Internet and e-mail.
  • Knowledge of CRMS, OCAN, and GAINS Q3 MI Ont is an asset.
  • Valid “G” Drivers license, relevant insurance and use of own vehicle.
  • A satisfactory Vulnerable Sector Screening (Police Check) report.
  • Current CPR/First Aid certification.
  • Ability to speak French or a second language reflective of the diversity of Region of Peel is an asset.
  • Proof of administered full-series Health Canada approved COVID-19 vaccination prior to start date. If unable to be vaccinated for reasons relating to protected grounds under the Code, completed medical or religious exemption must be provided.
