POSITION OUTLINE:Central West Local Health Integration Network (LHIN) currently operating under the business name Home and Community Care Support Services Central West is seeking a Regular Full-Time Learning Solutions Specialist to join the team to support the staff learning solution needs to facilitate the unique needs of the communities we serve.The Learning Solutions Specialist (LSS) is responsible for providing learning solutions that drives performance excellence at Home and Community Care Support Services Central West. The LSS will plan, design, deliver and evaluate learning solutions, facilitate formal training for new and existing employees, support new hires to successfully onboard (including performance coaching and support post training) and conduct on-going performance audits to proactively identify learning needs and solutions.The learning component will include learning and education needs pertaining to Home and Community Care systems including The Art of Case Management, the RAI- assessment tools, policies, procedures, programs such as customer service, orientation, case management services and other related Home and Community Care systems, programs and tools.QUALIFICATIONS:Three (3) to five (5) years’ experience in delivering learning solutions, facilitation and/or design in a patient centric environmentRegulated Health Professional preferredPrevious experience in applying adult learning principlesPrevious experience facilitating the learning of technical learning programs, ideally within a health care environmentProven excellent presentation and group facilitation skillsDemonstrated focus on providing innovative solutions to meet the needs of our learnersKnowledge of, and demonstrated experience in Case Management preferredKnowledge of, and demonstrated experience in RAI training preferredPost-secondary education in Adult Education preferredPrevious experience in designing, delivering and evaluation customer service training is preferredKnowledge of, and demonstrated experience in Change Management preferredKnowledge of and adherence to all Home and Community Care Support Services Central West policies, procedures and relevant administrative practicesKnowledge of Home and Community Care Support Services Central West client services strategies, objectives, priorities and programsPractical knowledge of Long Term Care Act, Substitute Decisions Act, Placement Regulations and Health Care Consent ActGood knowledge of community resources (e.g. services and programs)Strong understanding and commitment to quality programs and best practiceUnderstanding of the Home and Community Care Support Services Central West’s stakeholders, client service delivery frameworks and methods, and overall issues and priorities within the health care sectorExcellent working knowledge of client database systemsSuccessful completion of RAI-HC competency modulesProven facilitator experience with excellent communication skills (written/documentation and verbal).Strong relationship building skills and maintains positive working relationship with external and internal contactsExperience effectively working with multiple stakeholders to gain understanding, determine need and build learning solutionsExcellent critical thinking and decision making skillsSolid ability to use MS Office applications (e.g., Word, Excel, Outlook, PowerPoint, etc.)Highly detailed with strong organizational, time management, prioritization, planning and multi-tasking skillsFlexible, adaptable and responsive to changeAccountable for own actions and decisionsSelf-directed with the ability to work independentlyExcellent understanding of communication in education and trainingCourteous and respectful in all interactions as well as able to work positively and collaboratively within a team environmentStrong negotiation, problem solving, effective listening and facilitation skillsAbility to maintain confidential informationEmpathy to sensitive issuesAbility to communicate in French or another language an assetWho We Are:A mosaic of geographic and cultural diversity and home to over 922,000 local residents, Home and Community Care Support Services Central West plans, integrates, funds and monitors the local health care system for the regions of Brampton, Caledon, Dufferin, Malton, North Etobicoke and West Woodbridge.
