We are proud to be recognized, year after year, as one of Greater Toronto’s Top Employers and Canada’s Best Diversity Employers.Here you will work alongside a highly-committed and respected team that understands what it means to go beyond to meet the health care needs of our communities.POSITION: Social Worker – One (1) Temporary Part TimeDURATION: Approximately 15 months or until the previous incumbent returns.HOURS: Currently Days/Evenings, 8 hour shifts, including weekends and statutory holidays (Subject to change in accordance with the operational requirements).POSITION SUMMARY:The successful applicant will be part of a multi-disciplinary team responsible for the care of patients and families with ESRD, reporting to the Patient Care Manager, Dialysis, with professional accountability to the Professional Leader for Social Work. The dialysis social worker will be an integral part of the Social Work professional team, participating in yearly strategic planning initiatives and corporate professional development.QUALIFICATIONS:

  • Current registration, and in good standing, with Ontario College of Social Workers and Social Service Workers
  • Ability to provide coverage in Critical Care and throughout the organization as required
  • Masters degree in Social Work required
  • 3 – 5 years experience working with Renal patients
  • Knowledgeable in community resources (i.e. Kidney Foundation)
  • Membership in CANSW preferred
  • Knowledge and experience working with relevant legislation (ie: SDM Act, POA, Consent and Capacity, etc)
  • Knowledge of the psychosocial impact of chronic illness on the person and the family
  • Knowledge in Nephrocare preferred
  • Familiar with Ontario Renal Network (ORN) guidelines preferred
  • Excellent communication and interpersonal skills
  • Ability to work as part of a multi-disciplinary team
  • May be required to travel between and work at all hospital campuses
  • Excellent organization skills and ability to set priorities as required
  • Good critical thinking and problem-solving skills required
  • Excellent attendance, punctuality and work record
  • Computer proficiency preferred
  • Must be able to travel to all sites including satellite unit
