QUALIFICATIONS * Baccalaureate Degree in Nursing * Eligible for active practicing licensure as a Registered Nurse with applicable regulatory body in the province of Manitoba * Successful completion of the Manitoba Nephrology Nursing Course * Three (3) years related clinical experience as a Registered Nurse in Nephrology * Current Basic Cardiac Life Support (BCLS) Training, as delineated in Prairie Mountain Health Policy PPG-00002 CPR Training * Province of Manitoba Class 5 Drivers Licence, or equivalent from province of residence, and access to a personal vehicle to provide service within Prairie Mountain Health * Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology * Demonstrated organizational skills and ability to work independently * Demonstrated problem solving and decision making skills * Demonstrated flexibility to facilitate changes in techniques and procedures in a changing environment * Demonstrated knowledge and competence of skills and concepts related to the position * Demonstrated communication skills * Ability to respect and promote confidentiality * Ability to perform the duties of the position on a regular basis * Ability to respect and promote a culturally diverse population * Ability to work effectively and maintain positive working relationships with co-workers, clients and within interdisciplinary team POSITION SUMMARY: The acute care team is committed to providing program based care and service that focuses on the needs of patients and their families in an environment that fosters continuous quality improvement, interdisciplinary collaboration, professional development, and staff satisfaction. The Nurse IV recognizes and fulfills their professional responsibilities by practicing in a manner consistent with professional obligations to take actions as per the Canadian Nurses Association Code of Ethics for Registered Nurses and Registered Psychiatric Nurses, The College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses, The College of Psychiatric Nurses of Manitoba Standards of Psychiatric Nursing Practice, the scope of practice as defined by the Regulated Health Professions Act (RHPA), the Registered Psychiatric Nurses Act, and other Provincial and Federal legislation. RESPONSIBILITIES: Overview: Collaborates with the Provincial Renal Health Outreach Coordinator and the Interdisciplinary Health Care team in identifying, developing and implementing standards of care, as they apply to individuals with PRI. Liaises with staff, and assumes responsibility for selected clinical investigative tests and procedures on a select group of individuals. Functions in a collaborative manner as a member of an interdisciplinary health care team, in the planning, implementation and evaluation of health care in the clinical area. Consults and liaises with patients and their families, nursing staff, and other professionals for the resolution of complex care issues. Advocates on behalf of patients and their families. Participates in the development of new protocols in the PRI area. Participates in Quality initiatives/risk management activities. Assesses for, plans, implements, and evaluates learning needs of patients and their families. Participates in the development, implementation and evaluation of educational programs for patients and their families. Coordinates and provides patient education with members of the Interdisciplinary Team. Participates in the development of Renal Protocols and Renal Care Paths. Acts as a resource person by participating in the education of other health care professionals including educational seminars for community based clinics. Identifies, participates in, promotes and reports findings of research related to PRI. Utilizes relevant research findings to meet the needs of patients with PRI. Initiate and implement plans to upgrade own development through internal and/or external resources. Sets and evaluates individual objectives. Maintains necessary reports, records and statistics. Participates in the activities of professional nursing associations. Acts as a liaison between community based primary care Clinics and hospital based tertiary care Clinics. Adheres to all safety and health regulations and safe work practices. May be required to perform other duties and functions related to this job description not exceeding above stated skills and capabilities. Other duties as assigned.
