If you’re interested in driving excellence in care and service delivery, and seeking an unparalleled opportunity to lead and learn, partner and connect, care and be cared for, this is your home.What will you doReporting to the Senior Manager Quality, Risk and Patient Services, the Project Manager will provide day-to-day project management and oversight to the related organizational projects to execute solutions, attain key organizational performance improvements, and/or reduce organization business risk. This responsibility will include the planning, monitoring, evaluation, and management of projects and change initiatives to support strategic and business goals. The Project Manager will represent Home and Community Care Support Services Erie St. Clair, working closely with both internal and external partners from project initiation through closing, fostering and developing relationships that will promote an environment of collaboration, innovation, and the advancement of organizational endeavors.The Project Manager will provide expert advice to Home and Community Care Support Services Erie St. Clair staff and management on all matters related to projects and change initiatives. The Project Manager will champion the effective and consistent application of best practice policy and process. In addition, this role will enable the achievement of strategic objectives and promote a culture of continuous improvement.Key responsibilities will include:
  • Manage all aspects of the initiation, development, planning, execution, monitoring and controlling, reporting, evaluation, closing and sustainability of multiple high risk and complex priorities of each assigned project
  • Conduct readiness assessment and all due diligence activities, identify gaps, risks, and dependencies, itemize tasks and requirements as proxies for capacity and readiness
  • Establish clear communication and reporting on initiatives among leadership and project stakeholders
  • Facilitate process mapping for business processes requiring change
  • Participate in Business Process Improvement and facilitate the development standard operating procedures
  • Create sustainability plans of business processes for ongoing evaluation and monitoring
  • Apply high-level experience and expertise in the planning and development of solutions
  • Play a key leadership role in the creation and development of an effective, sustainable, and integrated health system. This includes: the analysis and synthesis of relevant knowledge and trends, interpretation and application of relevant theory and research; and, understanding and integrating principles of resource allocation and cost-effectiveness in system-level decision-making
  • Lead and support integration, coordination, and collaboration across health system partners, with the inclusion of quality improvement, approaches as the basis for change and improvement
  • Lead internal and external collaboration working toward the elimination of departmental and organizational silos
  • Lead, organize and facilitate project-related meetings, develop project reports, presentations, statistics and other related documents, as required
  • Facilitate the development and evolution of partnerships, facilitate consensus, manage relationships as appropriate, and work with all partners to achieve successful project delivery
  • Provide training, expertise, and support to Home and Community Care Support Services Erie St. Clair team members, leadership and external partners in the development, prioritization, and management of projects with a best practice project management methodology
  • Provide training, expertise, and support to Home and Community Care Support Services Erie St. Clair team members, leadership, and external partners with a best practice change management methodology
  • Monitor risks and issues through the identification process and assessment of perceived and actual risk situations, develop mitigation strategies and escalate to internal and external sponsors and stakeholders as needed
  • Take responsibility for all aspects of stakeholder engagement and planning as it relates to the project plan deliverables, project team needs, and requirements
  • Measure and report on project progress for bi-weekly reporting to senior leadership, and monitor and report on team performance to the Senior Manager Quality, Risk and Patient Services, project leads, and sponsors, as required
  • Manage conflicts and solve problems within the project team and with external project stakeholders, as required
  • Manage and support stakeholder relationships (internal and external) to develop strategies, action plans, and make recommendations/decisions for system-wide implementation associated with the specific projects
  • Support the development, implementation, and monitoring of complex planning processes associated with assigned projects
  • Work collaboratively to develop and standardize tools and processes based on best practice and organizational needs
  • Develop, maintain, and update project documentation and all project artifacts including project schedule and tasks assigned to team members
  • Promote a project management culture across the organization
  • Other duties as assigned
What must you have
  • Three (3) to five (5) years’ experience and demonstrated success in project administration/management, project and change management methodologies.
  • Three (3) to five (5) years leadership experience in a health care or similar environment with the proven ability to lead a high-performing team comprised of complex stakeholders.
  • 5 years’ experience with Microsoft Office tools (Word, Excel, PowerPoint, Outlook, etc.).
  • Demonstrated experience working effectively in partnership with a broad range of individuals and organizations of diverse backgrounds, preferably within the health care sector.
  • Demonstrated skills in planning and integration of complex multi-stakeholder initiatives.
  • Complete discretion with sensitive information and sensitive situations.
  • Valid Driver’s License and access to a reliable motor vehicle with appropriate business class liability insurance as travel throughout Erie St. Clair is required.
