In accordance with the Vision, Purpose, and Values, and strategic direction of Island Health (Vancouver Island Health Authority), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.Under general supervision, position is responsible for coordinating services for a caseload of individual clients in a program for adults with acquired brain injury, their families, friends, advocates, and support networks who receive funded and/or non- funded services from the Brain Injury Program, Vancouver Island Health Authority (VIHA). Conducts functional needs assessments and/or service requirement assessments and monitors individual client and funded support services and clinical support to clients receiving residential care and community rehabilitation services. Evaluates effectiveness and quality of services provided to clients, participates in the problem resolution process relating to individual and/or family crisis situations, facilitates mediation, and liaises with service providers, support networks, community advisory committees, and other professional personnel. Assists Regional Program Coordinator in service and/or contract development for the program and monitors existing contracts to ensure compliance with the Brain Injury Programs policies and procedures as delegated. Participates on interdisciplinary teams to ensure the provision of professional rehabilitation support services, assists in the production of transition-to-community plans and discharge planning, and participates in the development of rehabilitation and community support services supported by the Brain Injury Program. Travel may be a requirement of this job. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority in accordance with the service assignment and may require the use of a personal vehicle.QUALIFICATIONS:Education, Training And ExperienceBachelor in an Allied Healthcare Discipline. One (1) year recent related experience or an equivalent combination of education, training, and experience. Valid BC Drivers License.Skills And Abilities
