Our technology accelerates interventions, promotes preventative care, and allows you to deliver cognitive health services remotely from any device, while we handle scheduling, billing, and administration.Position Overview:We are seeking a compassionate and resourceful Case Manager/Clinical Liaison to support our clients in accessing essential resources, coordinating with healthcare providers, and navigating referrals. As a Case Manager/Clinical Liaison, you will play a pivotal role in ensuring our clients receive comprehensive care tailored to their unique needs. This role requires an individual who is organized, empathetic, and skilled in resource coordination.Key Responsibilities:

  • Resource Coordination: Assist clients in identifying and accessing community resources, including financial assistance, housing support, and social services.
  • Healthcare Navigation: Help clients find and connect with appropriate doctors, specialists, and healthcare providers.
  • Referral Management: Collaborate with clients, families, and healthcare professionals to facilitate seamless referrals to necessary services.
  • Advocacy: Advocate for clients’ needs by ensuring they receive appropriate services and support for their mental and physical health.
  • Educationamp; Guidance: Educate clients and their families on available resources and how to effectively utilize them.
  • Collaboration: Work closely with Total Life therapists and care coordinators to create a cohesive care plan.
  • Documentation: Maintain accurate and up-to-date records of client interactions, resources provided, and referrals made.

  • Bachelor’s degree in Healthcare Administration, Social Work, Psychology, or a related field.
  • Preferred 2-3 years of social work case management experience
  • Strong knowledge of community resources, healthcare systems, and social service programs.
  • Excellent communication and interpersonal skills, with a focus on empathy and client advocacy.
  • Ability to work independently while maintaining a high level of organization and accountability.
  • Familiarity with older adult populations and their unique needs is highly desirable.
  • Proficiency in using case management software and other digital tools.
