The Liaison Services Worker is responsible to provide assistance to and advocate on behalf of Clients with respect to Social, Health and Justice matters: complete forms, explain processes, connect individuals to resources and benefits, and coordinate opportunities to enhance social and health literacy.Duties:-Network and liaise with community organizations, Bands, and agencies in the Service Industry to promote a culturally informed resource base to provide the best possible outcomes for client and community.-Assess client needs to determine the basis for assistance.-Collaborate and coordinate the delivery of services to address needs.-Assist clients to complete appropriate forms.-Interpret policies, regulations, procedures and forms to clients.-Advocate on behalf of clients to ensure positive holistic outcomes.-Coordinate referrals and follow-up procedures to ensure client needs are being addressed.-Escort clients to referrals and resources to facilitate access and transition to services.-Coordinate opportunities to engage clients/participants in social/community development functions.-Maintain statistical and report information as required.-Attend training seminars, workshops and meetings as directed.Assistance with forms typically consists of completing applications for Health Cards, First Nations Status Registrations, Metis memberships, Income Tax Returns, CPP Benefits, Indian Day School applications.... The Liaison Services Worker also administers Fine Option & Community Service Order Programs for Flin Flon/Creighton & Denare Beach; helps with Elder programming and the Community Christmas Gift Program.Hours of work may extend beyond normal hours on an as required and occasional basis.Duties relevant to the position may be assigned from time to time.Job Type: Part-timePart-time hours: 20 per weekSalary: $20.00-$22.00 per hourBenefits:
