Work with supportive colleagues who are passionate about improving lives and champions for quality and continuous learning.Position SummaryAs a Social Worker, in our MASTRSamp; OASIS Programs, (programs for older adults), you will be an integral part of a collaborative, inter-disciplinary team focused on delivering seamless services and programs to our older adult clients and customers. Some evening and weekend work is required.What you #39;ll be doing• Conduct a comprehensive assessment for the client relevant to biopsychosocial history, diagnosis and ability to make effective use of treatment • Integrate the biopsychosocial history into the treatment plan • Develop and evaluate treatment plans in collaboration with the clinical team, community supports and families.• Facilitate group counselling sessions • Provide individual counselling in support of patient #39;s specific needs and goals • Ensure continuity of care is maintained through the transfer of information• Work with the clinical team to initiate community treatment orders • Establish relationships with key community supports• Discharge planning• Prepare referrals to appropriate community services• Document in a timely manner• Collaborate with the multidisciplinary teamWhat we #39;re looking for• Master #39;s Degree in Social Work (MSW) required• Registered member of OCSWSSW• Working knowledge of the Mental Health Act• Demonstrated experience working with addictions and concurrent psychiatric disorders • Trained in therapy interventions used in mental health (CBT, DBT, Motivational Interviewing)• Knowledge of standardized and non-standardized assessments• Experience in program development and delivery• Knowledge of local community social servicesAs part of our commitment to the health and safety of our patients, staff and community from COVID-19, subject to any accommodation required by applicable human rights legislation, Homewood Health Centre requires that all staff have received all required doses of a COVID-19 vaccine approved by Health Canada.Why work with usHomewood Health is Canada #39;s largest and leading service provider for medically focused, evidence-based treatment of mental health and addiction disorders.
