Registered Practical Nurse Number of Openings:2Division:VisitingFull/Part Time/Casual:Full TimeProgram:Visiting NursingRegular/Temporary:RegularLocation:HamiltonUnion:LIUNAHours of Work:*Days*Weekend rotation as per collective agreement*Posting Date:December 8, 2023Closing Date:December 15, 2023Job Posting Number:LIUNA 23-24French Language Skill Requirement:N/A POSITION SUMMARY:St. Joseph’s Home Care is dedicated to providing quality and compassionate care to individuals in their homes and/or alternative settings on a visiting or shift care basis.Registered Practical Nurses enhance the life of their clients by providing a range of care to clients with predictable outcomes, within the framework of SJHC’s policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario.Through their work, Registered Practical Nurses contribute to the healing and independence of their clients and promote the continual improvement of the overall health of the community. RESPONSIBILITIES:

  1. Functions independently in the community and provides quality nursing care, within their scope of practice, for clients and families who have a plan of care and for whom outcomes of care are predictable.
  2. Utilizes the nursing process effectively in order to provide high quality nursing care for clients and/or caregivers in all client situations.Applies current knowledge and research to practice
  3. Within the scope of practice for an RPN, follows plan of nursing care, including discharge plan. Through consultation and liaison with client, the LHIN, physician and other multi-disciplinary team members, evaluates and modifies plan of care on an ongoing basis.
  4. Provides nursing care primarily within the Hamilton Visiting Program, but also provides nursing care to clients within the Integrated Client Care Program if needed.
  5. Maintains current competence and participates in continuing professional development opportunities, obtaining advanced or specialized nursing skills to meet client care needs, when necessary.Carries out self-appraisals in accordance with the College of Nurses of Ontario Standards of Practice and Self-evaluation and the SJHC appraisal process.
  6. Acts as an advocate for clients/caregivers by interpreting information, supporting decisions and facilitating communications.
QUALIFICATIONS:
  1. Current RPN registration with the College of Nurses of Ontario with no restrictions on licence or hearings pending, AND
  2. Practical Nursing Diploma from an accredited college program with evidence of IV medication administration as part of the basic educational curriculum OR
  3. Practical Nursing Diploma from an accredited college program and IV medication administration certificate from an accredited college program if this content was not included in the basic diploma educational curriculum OR
  4. Practical Nursing Certificate from an accredited college program, a medication administration certificate and an IV medication administration certificate from an accredited college program.
  5. Current registration with the College of Nurses of Ontario.
  6. Basic Cardiac Life Support (BCLS).
  7. Ability to function to the full scope of practice as per the College of Nurses of Ontario.
  8. Experience conducting comprehensive nursing assessments (example: Mini-metal status, depression, pain, etc.)
  9. PPS, ESAS
  10. Can effectively use software for client documents, word processing and communication.
  11. Committed to customer service.
  12. Demonstrated commitment to professional development and continuing education.
  13. Good work performance and attendance record are essential.
  14. Demonstrated commitment to the values of Dignity, Respect, Service, Justice, Responsibility and Enquiry.
  15. Demonstrates the following competencies: Service and Mission Focus, Achievement, Teamwork, Learning and Innovation, Communication, Conflict Resolution and Resilience.
  16. Original and current (within six months) Police Check with Vulnerable Sector Screening.
  17. Valid Driver’s License. Proof of $1M automobile liability insurance. Able to drive locally and on the highway.
