We embed these values in the work we do in community every day.To learn more about CMHA NS, visit: www.novascotia.cmha.caABOUT PROJECT H.O.P.E: The Project H.O.P.E (Housing Outreach Peer Empowerment) team works with people in our community who have experienced homelessness, are currently homeless or are at risk of homelessness, while also experiencing mental wellness issues. People may self-refer or may be referred by community partners.JOB DESCRIPTION: The role of the Coordinated Access Project Manager is to act as the liaison for the homeless on behalf of community stakeholders represented in the Community Access Committee in the Annapolis Valley (Windsor/West Hants, Kings, Annapolis, and Digby Counties).They will be responsible for working closely with the committee to ensure that the resources in those communities are effectively leveraged to serve the needs of those who are homeless or are at risk of being homeless.KEY RESPONSIBILITIES: Key responsibilities would include, but are not be limited to:

  • Managing the By-Names List; a real-time list of homeless individuals that allows for prioritization and triage to services across Annapolis Valley (Windsor/West Hants, Kings, Annapolis, and Digby Counties.)
  • This includes ongoing coordination of the Coordinated Access Meetings
  • Respond to individuals, referral partners, and other entities regarding housing needs related to homelessness or at risk of homelessness
  • Record and document individual case information for referral to Housing Support Agencies
  • Utilizing Assessment Tools – assess needs of individual related to housing
  • Prepare internal and external planning and reporting reports
  • Develop and maintain an appropriate record system – utilizing HIFIS and other data gathering platforms.
  • Collect and analyze data as required
  • Provide oversight to Coordinated Access Data Collection, the assessment tool to evaluate housing needs
  • Represent Coordinated Access on municipal initiatives relating to homelessness and housing
  • Perform other related duties as required
LOCATION: This position requires the individual to be based in the Annapolis Valley, NS.WORKING CONDITIONS: This is a term position, working from 8:30 a.m. to 4:30 p.m. Monday to Friday (37.5 hours/week). This position functions in a combination of working in an office environment, with stakeholders, and working from home; within a flexible work environment and a workplace culture committed to personal wellbeing and psychological safety.COMPENSATION: Commensurate with experience. Range: $48,000 -$54,600 (fair market value within the sector)EQUIPMENT: The employer will provide necessary office equipment for the employee’s use while employed with CMHA NS Project HOPE. This includes a laptop computer, printer/scanner, and cell phone.A valid Nova Scotia Driver’s License and access to a vehicle is required.Reimbursement will be paid at a per/kilometer rate.The employee is responsible to have reliable internet service.QUALIFICATIONS/PERSONAL SKILLS & ATTRIBUTES:
  • Experience dealing with individuals in a ‘crisis’ situation

EDUCATION AND/OR WORK EXPERIENCE REQUIREMENTS:
  • Post-secondary education in the field of human services, or an equivalent combination of education and working experience
  • Experience working with marginalized populations (including but not limited to individuals experiencing homelessness, individuals who use substances, individuals with mental health issues)
  • Experience working with volunteer boards or community-based organizations would be considered an asset
  • Proficiency on the MS Office Suite
