People may self-refer or may be referred by community partners.JOB DESCRIPTION: The role of the Coordinated Access Project Manager is to act as the liaison for the homeless on behalf of community stakeholders represented in the Community Access Committee in the Annapolis Valley (Windsor/West Hants, Kings, Annapolis, and Digby Counties).They will be responsible for working closely with the committee to ensure that the resources in those communities are effectively leveraged to serve the needs of those who are homeless or are at risk of being homeless.KEY RESPONSIBILITIES: Key responsibilities would include, but are not be limited to:
