This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.ResponsibilitiesThe Patient Access Navigator is responsible for facilitation of access into and out of Mental Health & Addiction. the Patient Access Navigator will build working relationships, solve problems and support individuals while they learn to self-navigate the health care system. The Patient Access Navigator reduces barriers that prevent patients from getting treatment/support by identifying patient needs and directing them to sources of support. The Patient Access Navigator will provide support and referral to ancillary support services for individuals needing assistance for their mental health and/or substance use/gambling. The Patient Access Navigator will act as a liaison between community resources and MHA and will remain knowledgeable of community resources, as well as Mental Health & Addiction teams and resources to ensure the best care.Qualifications
