Your Opportunity:We are looking for a Licensed Practical Nurse with experience in patient-centered, trauma-informed, and recovery-based care for the AMH Outreach Team in Lethbridge, Alberta. This Licensed Practical Nurse - AMH Outreach position is based out of the Lethbridge Provincial Building and will provide concurrent support to individuals who require treatment in place for a variety of medical conditions including chronic disease, acute wound care and STBBI testing/treatment. Services will be community based and recovery oriented towards adults experiencing severe and persistent mental illness, addiction, and homelessness. You will work in collaboration with community partner teams, both AHS and non-AHS based to collaboratively provide care to individuals whose lives have been impacted by unstable housing, socioeconomic factors, and substances. Working within the LPN scope of practice, you will provide consultative assessments and intakes for clients to determine physical and mental health stability, complete thorough health assessments, treatment planning, management, and monitoring of both patient physical and mental health issues. You will demonstrate a knowledgeable and caring clinical practice that is grounded in addiction and mental health nursing from a recovery-oriented perspective; have strong communication and interpersonal skills; demonstrate time management, independent decision-making, critical thinking, and problem-solving skills; and an ability to foster positive relationships within a team environment are all expectations of this role.Description:As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
