Transitional Case Manager, Justice and CourtDirect Service - (OPSEU)Full-Time, Contract (Ending March 31, 2025)London, Ontario (London Zone)- Application Deadline: February 8, 2024 -The Transitional Case Manager will provide support to those involved in the criminal justice system, in partnership with the London Justice CentreWe believe in a support-first approach to those who come into contact with our justice system as a result of addiction or mental health concerns.You will work with clients involved with the justice system and who are experiencing mental health and addiction challenges to facilitate the transition from immediate mental health needs to an increased level of autonomy by promoting stability and empowerment.Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 3 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.Consider revitalizing your career and making a difference in your community by joining our team! WHAT WE #39;RE OFFERING:

  • What you #39;ll be making - Starting rate for this role is $26.48 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($29.28 per hour for Level 1 – Direct Service, Regulated):
    • Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($33.56 per hour for non-regulated, and $36.35 per hour for regulated)
    • For internal applicants, grid level placement will occur as per the Collective Agreement
  • When you #39;ll work – Hours are generally 8:30am to 4:30pm, Monday to Friday, with one unpaid hour for lunch. These hours may vary on occasion.
  • Your pension plan – Immediately eligible for optional in the Healthcare of Ontario Pension Plan (HOOPP)
HOW YOU WILL BE MAKING A DIFFERENCE:
  • Attend court and weekly pre-court meetings
  • Liaising with legal professionals e.g. lawyers, probation officers, social workers
  • Assisting with short-term case management by providing an urgent/emergent response to clients in need
  • Providing information and referrals
  • Conducting a variety of assessments (skills, functional ability, needs, barriers, etc.)
  • Empowering clients to identify personal goals and preferences to work towards recovery
  • Assisting clients to establish recovery-focused individualized rehabilitation plans
  • Supporting clients to navigate the addiction and mental health system and barriers they may encounter
  • Assisting and advocating for/with clients to obtain needed services and supports
  • Identifying, developing, and maintaining strong links with a wide range of community partners/resources
WHAT YOU #39;LL NEED TO APPLY:Police Vulnerable Sector Check RequirementsSuccessful candidates must have a Police Vulnerable Sector Check completed in the last six months.   • Degree or diploma in a health or social science discipline (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Valid Driver’s Licence and access to a vehicle to transport clients occasionally
  • Ability to attend work regularly
We prefer the following qualifications:
  • Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers, the College of Nurses of Ontario, the College of Registered Psychotherapists of Ontario, or the College of Occupational Therapists of Ontario
  • Knowledge of and experience working with individuals with a serious mental illness and/or addiction in a community setting
  • Excellent knowledge of community resources in London
  • Knowledge of/experience with working within the criminal justice system
  • Demonstrated ability to work with individuals living with multiple challenges related to involvement with the criminal justice system, mental illness, substance use, complex health challenges, street-level sex work, poverty, homelessness, abuse and trauma
  • Sound knowledge of computer skills and office technology, including electronic record database
  • First Aid/CPR certification
  • ASIST and CPI certification
