This position provides coverage to Lunenburg and Queens Counties. Third year competency in palliative care or equivalent is required for this position. A supportive interdisciplinary palliative care team is in place and consists of palliative care nurses, social workers, administrative assistant, and program manager.In addition to playing an important clinical role, collaboration and consultation to family physicians and medical specialists using a shared-care model.CCFP certified (or equivalent) candidates must have a certificate of added competence (or equivalent) in Palliative Care. FRCPC certified candidates must have either a certificate of added competence (or equivalent) in Palliative Care or Royal College certification (or equivalent) in Palliative Medicine.Preference will be given based on full time experience in palliative medicine. Candidates must be eligible for specialty licensure in Nova Scotia and must be able to independently commute to a variety of care settings served by our service, As the only palliative physician with the team, this means travelling across Queens and Lunenburg counties to see over 250 palliative patients in a year, at home and in three local hospitals, averaging about 1200 km a month.Care is provided in all care settings in the area including hospital, home, LTC, and assisted living residences. The Palliative Care physician is responsible to follow patients after initial consult with appropriate visits, phone support and collaboration with the patient’s other health care providers, in a manner that is most supportive of each family physician and patient. The physician works closely with the palliative care team. This team is an important part of the South Shore medical community.The Palliative Physician is currently consulted for 80-90% of all referrals to the Specialist Team, equating to on average, 85-95 patients on our caseload (including low priority patients) at any given time. In 2018 and 2019, we saw a ratio of about 5:3 for malignant vs. non-malignant referral, in 2020 this has changed to a ratio of 2:1.*Responsibilities*Responsibilities include but are not limited to:
