Overview: Due to continued growth, we are excited to be hiring a Bilingual Pharmacist to join our team at our dedicated Long Term Care Facility in Moncton.The Consultant Pharmacist is responsible for providing pharmacy services to contracted continuing care facilities, customers and medical practitioners. The Consultant Pharmacist will play a key role in achieving departmental excellence in the areas of patient focused care, customer service, workflow efficiencies, corporate pharmacy programs and contractual requirements of continuing care facilities based on corporate agreements. The Pharmacist will also provide guidance and supervision to Pharmacy Students, Pharmacy Technicians and Pharmacy Assistants. The Pharmacist will ensure that all patients are offered and receive professional clinical services. The Pharmacist will occasionally be on site at the continuing care facilities for many different reasons. The Consultant Pharmacists provide education for nursing staff and complete audits on stat boxes supplies (provided by the pharmacy), antibiotic use, antipsychotic use, wound care. Consultants provide quarterly reports to the continuing care facility administration, as well as reviews all medication errors related to their facilities. Actively contributes to an environment of employee and customer engagement.Patient Focused Care:•Provision of clinical services focused on continuing care contractual obligations (i.e. disease state management, medication reviews, full scope of practice, reporting, after-hours emergency service)•The consultant pharmacist sits on committees, does biannual medication reviews as part of a team with nurses, physicians, and NPs. The consultant pharmacists provide education for nursing staff and complete audits on stat boxes supplies (provided by the pharmacy), antibiotic use, antipsychotic use, wound care. Consultants provide quarterly reports to the continuing are facility administration, as well as reviews all medication errors related to their facilities.•Ensure prescription safety prioritizing technical functions to Pharmacy Technicians and Assistants as required•Initiate care plans, follow up and proper documentation•Collect relevant patient, drug and disease information•Identification of actual and potential drug related problems•Monitor medication adherence•Provide patient counselling for new therapy and provide education to patients/caregivers•Tailor counselling to the needs and concerns of the patient (i.e. literacy, hearing loss, privacy, etc.)Corporate Pharmacy Service Programs:•Responsible for understanding all Corporate Pharmacy Services offerings and providing each customer with the program that meets their health care needsOther:•Other duties as requiredJob Requirements: Technical Skills:

  • Above average communication skills (both oral and written) in French and English
  • Strong organizational skills and attention to detail
  • Self-directed and able to work independently to meet strict deadlines
  • Ability to be a team player in a fast paced environment
Education and Experience:
  • Bachelor of Science in Pharmacy from a recognized institution.
  • Must be a Licensed Pharmacist in the province of responsibility
  • Injection Certification
  • Previous experience in a LTC or retail pharmacy setting preferred
