Job Description

Community Mobilization Coordinator Position Summary:The coordinator position will facilitate the provincially led Community Mobilization initiative for Morden. This role will connect individuals, groups, and organizations to address social issues associated with health and safety, crime prevention, and community development. The role emphasizes the importance of empowerment-based interventions to strengthen support for at-risk individuals and enhance the problem-solving resources of the community. It requires multi-sectoral collaboration, the mobilization of community support resources, and coordinated service delivery for program participants.Details:Contract 2-year part-time with potential for renewal25-30 hours/weekSalary range: $30,000 - $35,000Roles and Responsibilities:

  • Address the complex needs of families in the community implementing a wrap-around approach that works collaboratively with program participants and extended support networks
  • Receive referrals from various partner agencies and determine if they are a fit and discuss with steering/screening committee
  • Liaise with referral sources to organize and facilitate Participant Engagement Meetings
  • Organize and facilitate multi-sector support team meetings for participants
  • Maintain the Participant Support Team Action Plan and assist the Support Team to implement the action items as appropriate
  • Encourage support team members to collaborate and monitor follow through
  • Follow up with referral source and other support team members
  • Meet with families to assist them in completing the Participant Post-Questionnaire
  • Liaise between Support Teams and the steering/screening committee as appropriate
  • Serve as a link to community support groups, agencies and broader community resources
  • Document system barriers identified during Support Team meetings, and communicate them to the Steering Committee
Communication & Community Relations
  • Network with existing and potential partner agencies that serve the Community from both the non- profit and government sectors
  • Facilitate coordinated communication between agencies where appropriate and support interagency collaboration through proactive communication
  • Provide presentations and resources to direct service staff in partnering agencies
  • Responding to public inquiries about the project that are not media related
Administrative Duties:
  • Attend steering/screening committee meetings and provide updates on referrals, open files, and service delivery presentations
  • Report to Steering Committee and attend Committee meetings
  • Coordinate communication with relevant stakeholders from government and non-profit sectors
  • Maintain Participant action plans and referral information
  • Maintain and process reporting data as per provincial funding requirements
