Job Description

About Us:Our company #39;s Rapid Advanced Caretm (RAC) model is tailored to the needs of frail and complex patients with higher acuity symptoms and chief complaints. By treating patients in place (generally in their home) we significantly reduce the need for the conventional acute care journey from ER to hospital admission to post acute care. Transforming the care experience - by bringing it home - radically improves patient and caregiver experiences, creates dramatic cost savings and avoids hospital acquired conditions.For the same population of patients we also offer a proactive Recovery at Hometm (RAH) program that decreases readmissions by 30% post acute discharge. When coupled with RAC, it offers a unique readmission prevention solution for the highest risk patient population.Summary DescriptionWe are hiring for our inaugural New Jersey team of passionate Physician Assistants and/or Nurse Practitioners who will perform in-home integrated medical services in partnership with supervising physicians (generally ED physicians). You will travel to a patient #39;s home or wherever they are, to provide extraordinary care and facilitate the coordination of patient care across a variety of providers and settings, to achieve integrated healthcare and improve patient outcomes. Within the company #39;s Flycar, you will have the point of care diagnostics, procedural and acute wound care supplies, and therapeutics, including IV fluid and medications, needed to ensure the highest quality health care outcomes.Key Responsibilities:

  • Working as part of a clinical care team to deliver Rapid Advanced Care and Recovery at Home in our patients #39; homes by:
  • Performing a problem focused assessment and various health assessments/screenings to ensure we are diagnosing and treating the factors leading to the acute symptoms
  • Ordering, performing and analyzing test results to support medical decision making
  • Performing POC diagnostic labs, phlebotomy for send out labs, point of care ultrasound (trained prior to performing in the field), EKGs
  • Actively providing therapeutic interventions, such as IV fluids and medications, splinting, suturing, acute wound care and arranging for appropriate follow up for the patient to ensure continuity of care
  • Organizing other mobile testing to ensure most appropriate therapeutic interventions
  • Integrating the patient #39;s (or health care agent #39;s) goals of care into the care plan applying, where appropriate, palliative principles to the patient #39;s care.
  • Using critical thinking skills and following evidence-based standards of practice.
  • Educating patients and families on how to best continue treatment of their acute illness and manage their health and well-being.
  • Communicating effectively with patients, family, the health care agent, primary care provider and all individuals involved in the patient #39;s care.
  • Real Time communication with referring clinicians and close the loop with these referrers once the patient has been evaluated and a plan initiated.
  • Effectively communicate with patients, caregivers and loved ones in diverse cultural, socioeconomic and age groups to interview patient, family members, and bystanders, and ability to discern deviations/changes patient #39;s condition and gives direction as appropriate and according to approved guidelines and protocol
  • Attending and actively participating in clinical conferences, training and huddles ● Identifying potential technical or patient care issues and work with leadership to make recommendations to enhance patient safety
  • Supporting interprofessional team and leadership to evaluate and measure the impact of new approaches that have been implemented
  • Supporting clinical guideline development, clinical pathways, disease management, transitional care management, drug use programs as identified and in collaboration with or under the guidance the medical director

  • A quick learner, willing to work in a fast-paced start-up where processes change regularly.
  • Competent in procedural skills including, but not limited to - IV placement, phlebotomy, I amp;D, , suctioning, foley catheter placement and care, G-Tube care, trach care, central line care, etc
  • Described as empathetic, compassionate and focused on the patient and family
  • Ability and willingness to work in a cross-functional team
  • Familiar with and able to demonstrate multicultural competence
  • Familiar with the socioeconomic, and educational issues facing the population assigned
  • Flexible availability, including nights and weekends
  • Must demonstrate unwavering commitment to improving the health of patients and bringing safety and comfort to communities.
Requirements
  • Current unrestricted state license as a Nurse Practitioner or Physician Associate in New Jersey
  • Minimum of two years of experience as a PA or NP in the Emergency Department, Urgent Care, Hospitalist, Cardiac, Pulmonary or other relevant areas
  • Current BLS required, ACLS certification preferred
  • Prescriptive authority and DEA
  • Valid driver #39;s license with clean driving record
  • Ability to walk up and down several flights of stairs easily
  • Must be able to maintain all required licenses and certifications, and must complete all required continuing education courses
  • Eligibility to work in the US without Company sponsorship for a work visa or employment-based green card
Special Considerations
  • Must be comfortable with unplanned care in a nonclinical environment.
  • Must be able and willing to work in patients #39; homes.
  • Must complete all required mandatory company learning models in a timely manner
  • Monthly scheduling will require flexibility and will require weekend and holiday hours.
Pay Range:$60-75 per hour*
