To help fulfill its mission, LOFT is hiring a Case Manager.Job SummaryThe Case Manager provides a range of services that supports individuals living with serious mental illness, addictions and physical health issues in York Region and South Simcoe. The Case Manager’s goal is to empower clients, thereby increasing their independence, dignity and self-respect in a safe and secure environment. This position liaises with other community agencies and professional services.The York Region/South Simcoe Community Case Management Program provides short-term support or intensive case management for transitional age youth, adults and older adults (also associated with LOFT’s Integrated Psycho-geriatric Outreach Program). We offer one-to-one support to assist individuals in meeting their basic needs (e.g., food, shelter, finances) and encourage the development of skills needed to live independently and improve quality of life.What You Will Do
