Your Opportunity:The Centennial Centre for Mental Health and Brain Injury (CCMHBI) has an exciting opportunity for a Social Worker in the busy Seniors Mental Health Program. Located in Ponoka, the Centennial Centre for Mental Health and Brain Injury is a State of the Art facility with National and International Recognition. It’s not a traditional hospital but rather a facility that provides specialized mental health and brain injury treatment and care. The campus of specialists and health professionals at the Centre is committed to integrating multidisciplinary care and treatment, ensuring the best outcomes for patients and their families. The Seniors Mental Health program provides assessment, treatment and rehabilitation programs for individuals, generally over the age of 65, with a psychiatric illness such as dementia and mood and/or thought disorders. The goal of the program is to assist each patient in gaining his or her optimal level of functioning in preparation for community integration. Specialty programming is offered in the areas of Mood and Thought Disorder, Psychosocial Rehabilitation and Enrichment and Specialized Dementia. As we are Patient and Family Focused, the Social Worker plays an important role in being a liaison between the family and the multidisciplinary team.Description:As a Social Worker I, you will be a member of a multi- disciplinary team that provides mental health services to individuals, families and/or groups at a community based facility. You will be responsible for the initial screening of patients by gathering and documenting client information, basic assessment and providing support to clients and their families short-term needs. You will liaise with and refer clients to appropriate community and government services and support, along with participating in case conferences and team meetings.
