Position: :Personal Support Worker - Community and Day Program (Part-time)Hours of Work: :Variable based on client demandReports to: :Home Support SupervisorDate Posted: : September 23, 2022Deadline:: Until position is filledPosition Summary:The Personal Support Worker (PSW) will provide:
  • personal home support services to community clients to support clients’ independence and safety and the maintenance of a safe living environment; and,
  • client supports with transportation to and from Day Programs
  • client supports within Day Programs
As a PSW at SPLC, you will provide one-on-one assistance to seniors in the comfort of their own homes/Day Programs to ensure they are living their best possible life, making a positive impact on their day-to-day health and well-being.****Areas Served:
  • East of Don Mills Road / West of McCowan Road
  • North of Ellesmere Road / South of Steeles Avenue East
  • Care service area is Scarborough
Responsibilities::1. Provides personal support services to community clients and clients at SPLC Centre and Terrace Buildings:
  • Provides personal care related activities of daily living: bathing, washroom assistance, changing incontinence products, dressing, personal hygiene and grooming and dental care according to the care plan
  • Assists with transferring, positioning of client, meal preparation, set-up and eating
  • Assists client with exercise programs under the direction of a health care professional according to the care plan, including walking with the client
  • Reminds clients to take medications (eg. blister pack)
  • Provide home making related activities: Safety checks, engage emergency services, reduce fall risks, engage clients in activities to reduce social isolation, how to eat healthy & prepare grocery lists, prepare meals including re-heating pre-cooked meals and light house-keeping.
2. Provides personal support services to Adult Day Program clients:
  • Provides personal care related activities in accordance with the clients service plan, including but not limited to: feeding, cleaning-up, toileting and hygiene, medication reminders.
  • Assists with cleaning and personal hygiene assistance after client incontinence
  • Escorts clients on SPLC vehicles during transportation to and from the program
3. Documents services and reports concerns and occurrences :4. Assist in daily program activities in the Adult Day Program:
  • Assists in program activity implementation and in encouraging client’s participation in activities
  • Assists with physical set up for program activities and daily routines
Education::
  • Personal Support Worker certificate
  • Valid First Aid and CPR
  • Certificate in Dementia Care an asset
Experience & Skills::
  • Experience working with frail or cognitively impaired seniors
  • Knowledge of health care, dietary needs or limitations of individuals with special needs as a result of ageing eg. Alzheimer disease, stroke, Parkinsons disease
  • Basic understanding of social, recreational or therapeutic programs an asset
  • Good communication and interpersonal skills
  • Ability to travel in the community (with reliable transportation)
  • Ability to physically assist clients with mobility, bathing, program activities, eating, toileting, and personal and hygiene care
  • Ability to maintain confidentiality and work in a multi-cultural and multi-linguistic community setting contributing to an inclusive and welcoming environment
  • Ability to speak English fluently and write clear and concise notes in English
  • Ability to speak Cantonese, Mandarin, Greek, Tamil, Korean language skills are an asset
Others::
    • Physician’s medical assessment required ( A TB test form will be provided)
    • Must be available to work day, evening, overnight or weekend shifts
    • Vulnerable Sector screening required
    • Proof of full COVID-19 Vaccination or Medical Exemption Certificate required
