Your Opportunity:This position is based in the North Zone community of Smoky Lake and includes specific assignment to the designated supportive living brain injury unit in Redwater. The Social Worker II provides clinical Social Work services and receives referrals from all sectors of the healthcare continuum including Acute Care, Home care and facility living streams. Working with a variety of interdisciplinary teams, this role may include the provision of direct clinical assessment and care, consultation, case management, education, program development and research. Services for the client and their families include bio-psychosocial assessment, treatment approaches using individual and family counseling methods, resource counseling and service delivery evaluation. The incumbent will demonstrate advanced organizational skills and the ability to re-prioritize quickly and respond to a wide range of requests and referral needs. This position will include travel to meet the needs of patients and families in both facility and home environments. The ability to understand program parameters and establish collaborative relationships with outside agencies is imperative to our patients (Alberta Works, AISH, OPGT, AB Seniors, PCNs, Foundations, Disability related programs).Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
