Job Description

Your Opportunity:This position is based in the North Zone community of Smoky Lake and includes specific assignment to the designated supportive living brain injury unit in Redwater. The Social Worker II provides clinical Social Work services and receives referrals from all sectors of the healthcare continuum including Acute Care, Home care and facility living streams. Working with a variety of interdisciplinary teams, this role may include the provision of direct clinical assessment and care, consultation, case management, education, program development and research. Services for the client and their families include bio-psychosocial assessment, treatment approaches using individual and family counseling methods, resource counseling and service delivery evaluation. The incumbent will demonstrate advanced organizational skills and the ability to re-prioritize quickly and respond to a wide range of requests and referral needs. This position will include travel to meet the needs of patients and families in both facility and home environments. The ability to understand program parameters and establish collaborative relationships with outside agencies is imperative to our patients (Alberta Works, AISH, OPGT, AB Seniors, PCNs, Foundations, Disability related programs).Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:Completion of bachelors degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).Additional Required Qualifications:Experience in the clinical area of medical social work required. Excellent written and verbal communication skills. Ability to establish positive relationships with a wide variety of patients and families. Ability to organize, manage time and set priorities in a fast-paced environment. Considerable initiative and judgement required. Experience working on an interdisciplinary team. Minimum of 2 years experience as a Medical Social Worker in an Acute Care or Community Health Care setting. Experience in the provision of facility-based care, particularly care of seniors and population with traumatic brain injury.Preferred Qualifications:Preference given to candidates with experience and knowledge in the area of brain injuries and working with vulnerable populations. Previous social work experience in a healthcare setting is preferred. Knowledge of the continuum of alternate decision-making options and related legislation.
