Registered Practical NurseNumber of Openings:1Division:CSSFull/Part Time/Casual:Full TimeProgram:TBEDSRegular/Temporary:TemporaryLocation:360 King Street EastHamilton OntarioUnion:LIUNAHours of Work:Nights- 11 pm -7Posting Date:June 12, 2023Closing Date:June 19, 2023Job Posting Number:LIUNA 23-11French Language Skill Requirement:N/APOSITION SUMMARY:The Registered Practical Nurse – Community Support Services, Transitional Beds Program (RPN - TBED) is responsible for providing quality and compassionate nursing care to individuals residing in the First Place Transitional Beds Program The RPN-TBED is accountable for enhancing the life of the residents by providing a range of care with predictable outcomes, within the framework of SJHC’s policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario.This role contributes to the healing and overall well-being of the residences, and promotes the continual improvement of the overall health of the Transitional Care Bed Program clients.The RPN - TBED works to support the Clinical Nurse Manager in the day to day clinical care operations of the program and works collaboratively with other care givers / staff, in order to help ensure the delivery of high-quality client care and effective clinical functionality.The RPN-TBEDS also supports the Manager – Operations, First Place in the day to day operational requirements of the program as necessary or requested.CORE DUTIES AND RESPONSIBILITIESClinical

  • Reviewing, updating and monitoring resident care plans in collaboration with Clinical Nurse Manager
  • Conducting symptom assessments using standard rating or scale systems (eg. ESAS) for all residents residing in the program on an urgent basis – as directed by Clinical Manager
  • Conducting Initial and ongoing pain assessment for residents who have reported pain.
  • Conducting weekly pain assessment using pain tool or pain ad for residents with ongoing pain management analgesic/opioid
  • Conducting Initial skin assessment for new admissions and reported skin impairment.
  • Conducting weekly skin assessment for resident with ongoing skin treatment.
  • Documenting and reporting client information according to SJHC policy, congruent with College of Nursing standards of documentation
  • Reporting resident symptom, assessment, prescription request to Primary Care Physicianon a timely manner. Follow up as required within 48 hours.
  • Seeking consultation from Clinical Nurse Manager in client situations that are beyond their scope of practice
  • Promoting the independence of clients and their caregivers
  • Acting as an advocate for clients and their caregivers
  • Interpreting information, supporting decisions and facilitating communications between clients, caregivers, and other clinical staff
  • Utilizing the nursing process effectively
  • Assessment, Diagnosis, Care planning, Implementation and Evaluation in collaboration with the Clinical Manager
  • Providing high quality nursing care for clients and/or caregivers in all clinical situations
  • Following plans of nursing care, within their scope of the RPN practice
  • Engaging in consultation and liaison with clients, their care givers, management, other staff, care partners, and/or external stakeholders.
  • Ensuring effective caseload management
  • Performing cost effective client care, including effective utilization of resources, through collaboration with management, Clinical Nurse Manager and other stakeholders
  • Participating in various SJHC team meetings and round table discussions
  • Demonstrating an evidence-based nursing practice
  • Facilitating improvements in the nursing care systems, practices or procedures
  • Communicating the needs and/or issues concerning assigned clients to Clinical Nurse Manager, other nursing team members and/or other stakeholders
  • Facilitating client/caregiver problem solving and resolution
  • Identifying, within their scope of practice, the complexity of care situations and utilizing the appropriate resources for resolution
  • Participating in various decision-making frameworks
  • Assume leadership/co-ordination responsibilities as required or requested
  • Other duties as required or requested
  • Transcribing new prescriptions into the drug record book.
  • Receiving medications on evenings. Ensuring prescription reflects on MAR.
  • Completing Falls Checklist
  • Completing Emergency Checklists
  • Participating in Clinical Rounds with Clinical Manager

KEY EDUCAITON AND EXPERIENCE
  • College level degree in Practical Nursing, required AND
  • Member in good standing with Ontario College of Nurses, required AND
  • Current and unrestricted RPN licence, required
  • Current First Aid with CPR certificate, required
  • Current Basic Cardiac Life Support (BCLS) certificate, required
  • Experience conducting comprehensive nursing assessments, required
  • Gentle Persuasion Approach certification, required
  • 3-5 years of previous RPN experience, required
  • Working knowledge of MS office software including excel spreadsheets, required
  • Previous experience working in a Community Care model or Transitional Bed setting, an asset
