Job Description

Programming and Case Management Coordinator Sussex Vale Transition House for SVTH and Second Stage HousingJob DescriptionThe Programming and Case Management Coordinator reports to the Executive Director and is subject to an annual written performance evaluation. This is a management position and is not unionized.Job PurposeThe purpose of the Programming and Case Management Coordinator is to provide support to theExecutive Director in providing Crisis Intervener’s direction and oversight as they deliver services to clients. This position also provides services to the SVTH Second Stage Housing project.Primary Duties and ResponsibilitiesPrimary duties include support with facilitation of in-house programming to staff and clients, collaboration with Crisis Intervener staff in case management planning and delivery, and Second Stage Housing project consultation and coordination.QualificationsBachelor of Arts degree in Social Work, Psychology or a related field required.10+ years of social work, DIPV sector experience and program facilitation.An equivalent combination of training and experience may be considered.Credentials (certificates, licenses, memberships, courses, etc.) Workplace Standard First Aid (required).Sexual Assault Crisis Intervention Training, Danger Assessment Training, Building on our StrengthsCompetenciesThe Programming and Case Management Coordinator must be capable of a high degree of attention to detail. Strong verbal and written communication skills are essential. The Programming and Case Management Coordinator must also have a familiarity of a broad range of educational and support services available in the community. The position requires a strong foundational knowledge base of the following:

  • Family/Intimate Partner Violence and its effects on women and children
  • Trauma, Mental Health and Addictions
  • Basic counseling skills/interventions
  • Goal setting and developing action plans to achieve goals
  • Organizing/Implementing/Facilitating Self Help groups and Life Skills Training Workshops
  • Program Development
Additional necessary skills include:

Major Duties
  • collaborate with Crisis Interveners to assist residents set goals and develop action plans to achieve the goals (case management guidance, facilitation, and support)
  • collaborate with government, non government agencies, volunteers, and residents
  • monitor and contribute to documentation of client-based work
  • provide individual support and basic counseling
  • monitor residents progress in attaining goals
  • make referrals to community-based services and public agencies
  • prepare and facilitate group programming / training
  • handle sensitive information in a confidential manner
  • other duties as required.
Rate of Pay ScaleRate of pay is $22.00 - $25.00 / hour and will be in accordance with individual experience and qualifications. Upon eligibility, group health benefit and RRSP matching benefits available.Working ConditionsThe work is based on a 24-hour work week. Flexible work hours may be necessary/available.This position may develop into a full-time position.*A negative result from both a criminal record check and Department of Social Department record check is required prior to employment with Sussex Vale Transition House.Job Type: Part-timePart-time hours: 24 per weekSalary: $22.00-$25.00 per hourBenefits:
