This employee works relatively independently and is expected to make primary decisions based on sounds assessment in accordance with The Public Health Act, both regional and PHN specific policy and procedures, the SRNA Standards of Practice, the Canadian Nurses Association Code of Ethics for Nurses, the Community Health Nurses of Canada - Standards of Practice, and PHN Discipline Specific Competencies. The Public Health Nurse translates knowledge from the health and social sciences to support health-enhancing behaviors of individuals, families and population groups through targeted interventions, programs, and advocacy. Inherent in this role are knowledge, skills, and abilities specific to health promotion, prevention, and protection. The PHN effectively uses the nursing process to provide safe, ethical and client /family centered health care.The PHN role includes:- Health assessment, health planning, health education, supportive counseling, preventative and screening measures, referrals and evaluation as appropriate.- Provides services to individuals, families, groups and communities with attention to diversity and the social determinants of health.- Immunization for all age groups, influenza program delivery, and health promotion initiatives.- Communicable Disease Control outbreak management, notifiable disease follow up.- Maternal/Infant Health including postpartum support, lactation support and education, prenatal education, and high risk assessment of mother,babies, and families in the home environment.- Screens infants and preschool children for specific delays and defiFor more information on this job, please visit the employer #39;s website.
