Job Description

Join our team in our mission to continue to put patients and families at the centre of everything we do, in the role of Health Information Management Professional.POSITION SUMMARY:Reporting to the Director, Information & Performance, the Health Information Management Professional will perform technical duties related to health information management. Additionally, organization, coordination and generation of reports related to technical analysis of health information is require. All duties are performed in accordance with Providence Healthcare’s Mission & Values.MAJOR RESPONSIBILITIES:

  • Maintaining patient confidentiality of patient/resident information and record security at all times;
  • Coding the appropriate final diagnoses from the summary sheet and from the chart, using I.C.D. 10. CA coding after the chart has been completed by the physician for discharges in rehabilitation;
  • Electronic abstracting of pertinent patient information from the chart using the applicable software on a daily basis (for rehabilitation discharges);
  • Completion of monthly/yearly statistics for Providence Healthcare;
  • Responsible for ensuring that monthly abstracts are completed within a specified time limit;
  • Preparation and processing of medical audits as deemed necessary by the Medical Advisory Committee or sub-committee, physicians, or Health Information Management Staff;
  • Quantitative and qualitative analysis of each chart on discharge; assembling all loose sheets in the listed order and binding of charts into the folder; checking each chart for deficiencies such as no discharge order, final diagnoses on summary sheet for rehabilitation discharges; final note, signatures, etc. For noted deficiencies, charts follow up with appropriate physician(s) responsible for the necessary completion of the chart;
  • Attending to medical and legal correspondences and release of information;
  • Revision of procedures to update various technical duties ( update the departmental as well as assist in updating organizational policies & procedures mainly related to privacy, confidentiality and release of information areas. With every change, there will be new responsibilities/duties added to each staff member with regards to monitoring processes) when necessary;
  • Involved in testing all the modules of Winrecs and upgrades on an ongoing basis;
  • Involved in developing/building the Meditech dictionary for the ADM/MRI/ABS;
  • Continuously involved in customer support especially in the area of confidential release of health information;
  • Assist in the chart audit process as well as quality assurance of ICD-10 coding/NRS coding;
  • Continuously involved in CCRS/NRS audit process;
  • Edit the transcription notes (this service is outsourced to MedQuist Canada);
  • Ensure that all charts of discharged patients/residents are brought to the Health Information Management Department within 24 -48 hours of discharge for the purpose of completion;
  • Transcription of minutes of the medical Advisory Committee and sub-committee meetings as required, and circulation of minutes to all committee members.
QUALIFICATIONS/SKILLS REQUIRED:
  • Two (2) year Diploma in Health Information Management obtained from a currently recognized academic College by Canadian Health Information Management Association;
  • Certified as a Health Information Management Professional CCHRA(C);
  • Current registration with the Canadian Health Information Management Association;
  • 3-4 years of experience as a Health Information Management Professional handling the work area listed above in a similar facility;
  • Excellent working knowledge of Microsoft office package- Word, Excel, Access, WinRecs V 8.3, and Meditech;
  • Awareness of the various legal aspects governed by the following:
    • The Public Hospitals Act
    • Charitable Institution Act
    • Coroner’s Act
    • Mental Health Act
    • Substitute Decisions Act
    • Health Care Consent Act
    • Privacy Act
    • Personal Information Protection and Electronic Documents Act
    • Guidelines to the Code of Practice for Health Records
    • Guide to Hospital Accreditation.
    • The Canadian Hospital Law
    • Report to the Commission of Inquiry into the Confidentiality of Health Information
    • Any other relevant legislation to come into effect;
    • Very good attendance record;
    • Good performance record
