Position Profile:Reporting to the Manager, the ‘Patient, Family Facilitator’ supports the well-being of our patients and their loved ones by assisting with the booking and facilitating of virtual e-visits, distributing personalized messages and the scheduling of essential care partner visits. As a highly organized and customer-service focused individual, you will be responsible for:Main Responsibilities:e-VisitsResponding to inquiries and coordinating requests for virtual visitsCollaborate with clinical units to schedule virtual visitsMeeting in person with in-patients to set up and complete the zoom e-visit; Ensuring the appropriate disinfection of electronic devices and equipment in adherence with necessary policy and procedures related to Personal Protective Equipment (PPE) and Infection Prevention practicesEducating , troubleshooting and providing support with regards to zoom/iPad device technologyObtaining and track feedback from patients, families and staff on experience with virtual visitDelivery of devices, ear plugs and/or device stand to patient care areas / bedsideErase meeting data, manage central email box, update/manage excel booking / worklistCoordination & Scheduling of Essential Care Partner VisitsResponding to inquiries and requests for Essential Care Partner visitsCollaborate with clinical units to schedule Essential Care Partner visitsEnsure Essential Care Partner visits meet the criteria as outlined in hospital policyCommunicate information about scheduled visits with Essential Care PartnerDocument scheduled visits utilizing e-tool/technologyWell Wishes DeliveryPrinting and delivery of Well Wishes emails to Team Leaders/Unit Clerks on Patient Care UnitsQualifications:College diploma required or University Degree preferredPrevious experience working with computer software, apps and hand-held technology including excel, word, iPad, iPod and zoomWell developed organizational skills with the ability to work in fast-paced environment and prioritize tasks appropriatelyExperience and sensitivity in working with patients and familiesExcellent communication, interpersonal and customer-focused skillsExcellent problem-solving, negotiation and conflict management skillsProven ability to work independently and show initiativeAbility to work effectively and safely when dealing with stressful situationsSatisfactory attendance recordWillingness to wear personal protective equipment (PPE) were requiredThoroughness and attention to detailFlexibility and adaptabilityDemonstrated commitment to and understanding of the mission and values of Unity Health Toronto
