Job Description

Overview:MyoHealth Rehab and Wellness Centre has an exciting opportunity for a Social Worker to join our team of health care providers. We are a private clinic that specializes in the treatment of clients injured as a result of motor vehicle accidents. Joining this multi-disciplinary rehabilitation clinic in Scarborough offers the right individuals the opportunity to collaborate with a team in providing exceptional care to our clients in a culture that is client-focused, diverse, inclusive and encourages personal growth.Roles and Responsibilities:

  • Provide clinical consultation, assessment, and counselling related to patients with complex medical needs
  • Preparation and documentation of psychosocial assessments, treatment plans and patient clinical progress reports
  • Collaborate with the social work team in providing complete patient care including arranging any resources required; consulting with other clinical staff to contribute to overall patient care
  • Liaise with colleagues, clinical staff, management and external providers including adjusters, lawyers and other members of the community as appropriate
  • Provide resource navigation within health care services and the community for patients and/or their families
  • Assist in the development and implementation of policies, priorities, and procedures to streamline day to day activities
  • Commit to professional learning development activities as requested by Supervisor and/or the Clinic Director in a manner that is consistent with the duties of the role
  • Engage in other duties as may be assigned by the Supervisor and/or the Clinic Director
Qualifications and Experience:
  • Masters Degree in Social Work required
  • Member in good standing with the Ontario College of Social Workers and Social Service Workers for practice in all applicable clinical areas
  • Previous experience working in a health care setting/private clinic (preferably 2 years)
  • Excellent assessment, clinical interviewing and problem solving skills
  • Excellent communication skills (oral and written), negotiation, mediation and organizational skills
  • Demonstrated ability to work collaboratively in an inter-professional team environment as well as independently
  • Knowledge of the health care system and community resources
  • Superior ability to manage multiple tasks, set priorities in a highly dynamic and quick-paced environment.
  • Strong computer skills with knowledge of Adobe and MS Office
  • Sound judgement, tact and ability to maintain strict confidentiality.
  • Experience with insurance procedures and trauma and MVA patients would be a strong asset
  • Performance-oriented
