Job Description

Union Affiliation: Non UnionSocial workers in health care provide in-depth bio-psychosocial assessments, diagnostic, treatment and evaluation services for individual, family group and couples. Counseling, discharge planning, resource referral, consultation and collaboration, teaching, research and patient/family education.The Social Worker in Critical Care at St. Joseph’s Health Centre is an integral member of the interprofessional team that serves patients and families of patients who are facing life-threatening illness or injury. In addition to the above mentioned activities, Social Workers in Critical Care settings specialize in family supports such as identifying and working with Substitute Decision Makers, supporting end-of-life decisions and providing grief supports where appropriate.DUTIES & RESPONSIBILITIES:

  • Able to work effectively as a member of an inter-professional team, collaborating with other disciplines to provide comprehensive patient care
  • Actively contributes to the academic mandate of the organization as an educator, researcher, resource or support for staff, students and members of the academic and professional community
  • Identifies and addresses educational needs for patients and their families
  • Demonstrates commitment to ongoing professional development in order to continue to provide optimal patient care
  • Is committed to, and participates in, ongoing quality improvement in the organization
  • Provides patient care in accordance with all professional and organizational standards
  • Demonstrates understanding of the social determinants of health and knowledge of how to access and link patients to appropriate community resources and services such as: OW, ODSP, Trillium, Community Housing, CCAC etc. where appropriate
QUALIFICATIONS:
  • Must have a Masters Degree in Social Work (MSW) or Equivalent
  • Eligible to engage in student education with academic partners.
  • Must be currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas.
  • Current CPR certification
  • Excellent communication, critical thinking and interpersonal skills
  • Proven flexibility and adaptability in a fast-paced environment
  • Excellent time management, organizational and prioritization skills as relates to patient care
Must be willing to work evenings and/or weekends as directed by the Program.
