St. Joseph’s Health Centre, Unity Health Toronto has opened a 30 bed Reactivation Care Unit located at Humber Church site (200 Church St.; near Jane Street south of the 401). The Reactivation Care Unit provides care for adult and geriatric patients who require therapy, reactivation and transition planning in order to transfer to the community and/or other supportive environments.For more details on the location, please follow this link:https://www.google.ca/maps/search/humber+church+site/@43.6944271,-79.5441726,13z/data=!3m1!4b1This unit focuses on reactivation and transition planning for patients who are waiting transfer to the community and/or other supportive environments. Working as a part of a dedicated, interprofessional team, you will deliver patient centred care designed with best practice principles. This team works closely with patients, families, and caregivers to provide reactivation therapy with a goal of enhancing and optimizing patient’s functions and engagement in daily routines.The Team Leader will provide high quality assessment, prioritization and organization of patient care for a variety of medicine patients within an Interprofessional Collaborative Practice philosophy. He/She has a working knowledge of resource management and has the ability to manage and resolve Access and Flow issues and constraints on an ongoing basis. The Team Leader is committed to working with the Patient Care Manager in providing high quality, safe care to patients on a daily basis. The Team Leader is a patient advocate with a patient and family centred approach. He/she uses clinical expertise to provide leadership, supervision and mentorship to students and staff on the unit. This leader is a role model with advanced problem solving and leadership skills. Being part of this team means being part of an exciting new initiative to help us support patients along their continuum of care and to shape how care is delivered.Qualifications:Current registration with the College of Nurses of Ontario.Current BLS/CPR certificate required.A minimum of two years recent RN experience with Medicine patients required with demonstrated practice excellence.Recent leadership experience required.Recent experience with opening new units and bed moves preferred.Recent experience with actively facilitating discharges with patients, families, and team including but not limited to leading and participating in family meetings.Recent experience in program/service development and team development.Recent experience with active management and resolution of Risk reporting.Demonstrated understanding and champion of patient and family centred care.Demonstrated history of initiating and supporting organizational change.Experience with bed management and utilization management preferred.Excellent communication skills.Demonstrated critical thinking and problem-solving abilities.Demonstrated skills in complaint management and conflict resolution.Demonstrated commitment to ongoing professional development and learning.Computer skills required (Word, Excel, PowerPoint, Sunrise, Electronic Patient Record and Email)Satisfactory Attendance Record.Quick Apply
