Ultimately, this is where you want to be.In partnership with the Regional Stroke Steering Committees the District Stroke Program Coordinator, is accountable for providing leadership, development, implementation, and evaluation of a stroke system of care for York region based on evidence and best practices. The Coordinator will work with local partners to create and sustain an integrated regional program which coordinates the infrastructure required for a full continuum of stroke care across all spectrums (i.e., promotion, primary, secondary, acute care, rehabilitation and home care). Through collaboration and engagement amongst all stakeholders, including: Mackenzie Health leadership, Central East Stroke Network, Stroke Planning & Care Council, Ontario Health Teams, and Central LHIN partner hospitals - Oak Valley Health (formerly Markham Stouffville Hospital), Southlake Regional Health Centre, North York General Hospital, and Humber River Hospital, the Coordinator will provide strategic and operational leadership for all components of the York Region District Stroke Program. Responsible for promoting and sustaining excellence in interdisciplinary stroke care, the Coordinator will support employee learning, growth and evidenced-based practices, and will be accountable for the coordination of human and financial resources (as required) to provide quality patient care across the continuum.Contribute to safe, quality care by:
