Job Summary and RequirementsThis posting is for two permanent full time Social Worker positions, working 75 hours bi-weekly.Community Outreach Services (COS) provides an Intensive Case Management model of care to individuals experiencing homelessness, live with substance use challenges with or without mental health problems, often have histories of trauma and other complex care needs and experience barriers in receiving care that interfere with their ability to live safely and independently in the community.COS is a collaborative multidisciplinary team consisting of social workers, outreach supports workers and a community liaison coordinator. The team works alongside their clients to provide a range of services such as street outreach, advocacy and brokerage of services including access to health, housing, income and other identified community resources.
