Consistent with the principles of primary healthcare, provides access to first level basic healthcare within the primary care nursing scope of practice for individuals, families, groups and communities. These providers act as the first contact and principal point of continuing care for patients within a healthcare system. The Interprofessional Team Demonstration Initiative (ITDI) is a Primary Care renewal initiative of integrating Regional interprofessional staff within private fee-for-service clinics throughout the province to support delivery of team based Primary Care. As a member of an interdisciplinary team, the Primary Care Nurse provides comprehensive healthcare with an emphasis on healthy living, illness prevention (primary and secondary), health education, chronic disease management, clinical intervention, and palliation.
