Case Management Clinician - PsychogeriatricClassification: Case Management ClinicianFull-Time, Permanent – October 2022 startMulti-Site Accountability; Oxford Catchment Area- Application Deadline: August 12, 2022 -We believe that everyone has the right to live with dignity, autonomy and independence in our community.As a Case Management Clinician you would work one on one or in groups with individuals who choose our program to meet their needs. You will support individuals as they improve their health and quality of life in the face of Mental Health and/or Addictions concerns and the accompanying barriers they experience. The SOAR Case Management Clinician assesses the person’s needs, strengths, and supports to co-develop goals and a customized recovery and support plan. You would work from a trauma informed perspective as part of a team and part of a community to advocate, challenge stigma, and prevent harm.The Case Management Clinician acts as a special resource in psychogeriatric care, including concurrent disorders; provides consultation and formal teaching; is responsible for the provision of clinical services to the psychogeriatric client through assessment, treatment, monitoring and directing care; act as liaison with community agencies and care providers; and assists in community development activities.*Consider revitalizing your career and making a difference in your community by joining our team!*Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months.   • What youll be making - $31.74 per hour
  • When youll work - 70 hours over a two week period, flexible time schedule

HOW YOU WILL BE MAKING A DIFFERENCE:
  • Establish a working and supportive relationship with clients
  • Assess clients’ mental, physical and emotional functional abilities and needs and investigates client’s present resources
  • Liaise with other professionals and with the community for the benefit of the client while adhering to the principle of confidentiality.
  • Advocate for client rights as necessary or ensure self-advocacy
  • Support and assists family members with issues related to the client’s mental illness
  • Devise a service plan with the client, establishing goals to meet determined needs
  • Provides skill teaching, concrete counselling, and support to clients to assist in goal achievement
  • Monitor symptomology, medication, and activities of daily living to help improve individuals’ ability to function on a day to day basis and within the community
  • Provide assistance with financial management.
  • Provide crisis support and crisis prevention planning
  • Provide vocational and occupational support
  • Provide support to client’s on other workers’ caseloads as needed
  • Work in collaboration with partners and various CMHA teams including Housing, Court, Crisis, and Concurrent Teams to provide timely interventions
  • Provide discharge planning
  • Maintain current and accurate client documentation records as per agency policy
  • Ongoing professional skill development/continuing education towards the delivery of mental health services.
  • Ensure that all program/caseload information is handled according to organizational standards of privacy and confidentiality
WHAT YOULL NEED TO APPLY: Vaccination RequirementsCMHA Thames Valley Addiction & Mental Health Services requires that all staff members be fully vaccinated against COVID-19, with the rare exemption of those individuals who cannot be vaccinated due to approved exemptions (medical reasons or religious grounds as recognized in the Ontario Human Rights Code).   • RPN, RN, OT, BSW or MSW
  • Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers, the College of Nurses of Ontario, or the College of Occupational Therapists of Ontario
  • Experience and detailed knowledge of serious mental illnesses usually gained by several years of working with this population
  • Experience in working in a community-based setting with an extensive knowledge of social assistance programs and both generic and specialized local social support networks is essential
  • Crisis Management/Intervention Skills
  • Knowledge of Psycho-social Rehabilitation
  • Extensive knowledge of Services in Oxford County
  • Ability to develop short and long term treatment plans in conjunction with the individual
  • Planning and organizational abilities
  • Ability to advocate and coordinate service access
  • Extensive knowledge of the support/treatment services network in Oxford County and of regional services commonly accessed by the Program
  • Excellent communication skills
  • Strong active listening and verbal communication skills
  • Ability to build relationships
  • Computers skills
  • Satisfactory Criminal Records Check and Vulnerable Sectors Screening
  • Valid driver’s license, access to a reliable vehicle, and vehicle insurance in compliance with CMHA Policy
  • Demonstrated ability to attend work regularly
We prefer the following qualifications:
  • Experience working with seniors and detailed knowledge of their needs
  • Experience with P.I.E.C.E.S framework
  • Experience with MoCa assessments
  • Experience in crisis intervention/management and crisis resolution
  • Experience working with individuals with an ABI or Development Disability
  • Experience in the treatment of Addictions
  • Extensive knowledge of pharmaceuticals used in the treatment of mental illness & experience in monitoring/administration of medication
  • Knowledge of legal system
  • Current CPR and First Aid certification
  • ASIST certificate
