DescriptionPeaceHealth is seeking a RN Home Care - Home Care Admin for a Full Time, 1.00 FTE, Day position . The general salary range for this job opening at PeaceHealth is $39.21 - $74.43. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc.Hiring bonus and relocation assistance may be availableResponsible for providing direct skilled nursing care and coordination/management of patients admitted to home care/clinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation.DETAILS OF THE POSITION:- Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services.- Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.- Performs physical assessments, develops, coordinate, modifies and manages written individualized plan of care, as ordered by the primary physician, to meet patient #39;s long and short-term medical goals and appropriate/timely home health outcomes.- Develops and clinically manages the Home Health Aide #39;s care plan according to the patient #39;s evolving personal are needs.- Coordinates care, collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.WHAT YOU BRING:- Graduate of an accredited School of Nursing is required; Bachelor #39;s Degree preferred.- 2 years of acute clinical or home health/hospice nursing experience- Licensed as a Registered Nurse in state of Washington- Drivers License and proof of auto insurance- Basic Life Support certificationPeaceHealth is committed to the overall wellbeing of our caregivers: physical, emotional, financial, social, and spiritual.
