We are a fully delegated Aboriginal Agency providing programs and services to Indigenous children, youth, and families on and off reserve within Stó:lō Territory in the Fraser Valley between Yale First Nation and Surrey BC.Our programs and services are created and delivered with the support of our Elders Advisory Council with special attention on strengthening culture and identity, while honoring the strength of families.

Youth Transitional Coordinator-1307Regular Full Time Position – (Global)Surrey Family Strengthening Team 2 (Permanency Team)Salary- $42,187.53 to $49,145.73 per annum (35 hrs per week)POSITION SUMMARY:The Youth Transition Coordinator’s primary focus is to work directly with youth in care, community stakeholders, and staff of the agency in four (4) service deliverables:
  • Conducting Youth Transition Conferencing.
  • Organizing activities for youth in care to have meaningful input to the agency leadership.
  • Hosting and facilitating groups and programs for youth in care.
  • Supporting life skill development for youth in care.
The top priority of the Youth Transition Coordinator is to work directly with youth in care to organize and facilitate Youth Transition Conferencing. Youth Transition Conferencing is a process where youth in care design and host planning conferences with their support networks that will assist them in having sufficient life skills and supports once out of care.The Youth Transition Coordinator may also work directly with youth in care to ensure that their voice is reflected in the design, implementation, and ongoing support of a Youth Council as well as organizing the annual Youth Visions and Voices Forum. By working collaboratively with staff and agency stakeholders the Youth Transition Coordinator may also lead the development and implementation of programs and groups for youth in care.Xyólheméylh is proud to have an Elders Advisory Council to ensure that culture is embedded throughout the agency ensuring the goals, missions and values are upheld in a good way. This position is expected to work in LÉTSÉ MÓT TÉ SQWÁLEWEL - “With a good mind, a good heart, and good feelings.”EDUCATION, EXPERIENCE & KNOWLEDGE REQUIRED:Degree or diploma in Human Services is preferred, however significant cultural knowledge and a demonstrated ability to develop respectable relationships, with On-Reserve Indigenous communities, and equivalent personal experience may be accepted.Ability to organize and conduct work with limited supervision.Excellent verbal and written skills.Ability to effectively engage with youth and facilitate focus groups and forums with diverse groups.Recent work experience in community development and/or project management.Knowledge of Indigenous communities, organizations and service providers is an asset.Knowledge and experience working with high-risk Indigenous Youth is an asset.Valid BC Driver’s License (no restrictions); reliable vehicle with adequate insuranceCriminal Records Check is a requirementCONFIDENTIALITY:FVACFSS employees are expected to be familiar with and must comply with the expectations of confidentiality as outlined in the Agency’s Human Resources Policy Manual and other policies and operational manuals, AOPSI and the B.C.
