Visit us today and check out www.novascotia.com to see why more people from across the globe are moving here.ResponsibilitiesReporting to the Manager Palliative Care, Seniors’ Clinic and Spiritual Care and working in collaboration with the Palliative Care Team. This position provides a full range of social work services to clients and families, that encompass clinical social work; counseling; social work consultation and education services for Palliative Care clients and families in community setting. The Palliative Care Social Worker will act as an advocate for those with a life-limiting conditions assuring basic human needs are in place while respecting the client’s right to self-determination while upholding human rights. The Palliative Care Community Social Worker will be an active member of the collaborative inter-professional Palliative Care Consultation Team and will work in shared care with each client’s Primary Health Care Team. This position will be based out of the Cumberland Regional Health Care Centre and provide services throughout Cumberland County, offering both a community based and hospital based setting.Qualifications

  • Must be registered (or eligible for registration) and a member in good standing with the Nova Scotia Association of Social Workers (NSASW).
  • Bachelor of Social Work degree (BSW) from an accredited and approved School of Social Work/University Program
  • 2 to 3 years experience in hospital based Social Work practice is required
  • Must have the ability to work effectively as a team member of an interdisciplinary hospital based health care team and assume positions of leadership as required.
  • Must possess strong problem solving skills, the ability to think critically and have strong decision making abilities.
  • Demonstrated competency with computer applications; Microsoft Word; Excel; PowerPoint; email and Meditech.
  • Travel is required. Valid driver’s license and access to reliable transportation is required.
  • Competencies in other languages an asset, French preferred.
