Description of Duties and AuthorityThe Intake Service Coordinator reports to the Manager of Planning for the duties related to general inquiries, accurately processing all client information, database management, gathering statistics and effective waitlist management. While offering exceptional customer service to applicants and community members.Program and Organizational ManagementIntake Responsibilities:
  • Process all client referrals and complete the intake cycle for all applicants.
  • Determine eligibility according to Ontario Health and Ministry guidelines.
  • Respond to all service inquiries in consultation with the Manager of Planning.
  • Update and maintain electronic files and Database to ensure accurate records.
  • Maintain waitlists, to ensure applicant information is current and accurate.
  • Provide information to applicants on external resources and assist with referrals as required.
  • Participate in team meetings, handle correspondence, and share meeting responsibilities.
  • Contribute to a team approach to problem solving, risk mitigation as well direct contact with clients, staff, community partners ILS management team.
Referral Responsibilities:
  • Maintain awareness of the various government and community programs and resources designed to assist seniors and individuals with disabilities to participate safelyamp; independently in their community and within their homes.
  • Complete referrals for current/active clients (ie. OT, PT, Nursing, PSW) through Health Partner Gateway.
  • Network with partner agencies/groups, and joint problem solving to address gaps.
Assessment and Service Coordination Responsibilities:
  • Arrange and conduct initial home assessment using standardized assessment tools, i.e., interRAI tools and all ILS related documentation.
  • Ensure timely admissions to ensure Ontario Health and Ministry targets are met.
  • Assist in the development of a care plan and identify long and short-term goals with the clients and caregivers while keeping client’s independence as a top priority.
  • Work closely with the Client Service team members to plan and coordinate the delivery of services for new admissions and mitigate risk.
  • Complete a Risk Assessment of the client’s physical environment and note hazards that will require correction to ensure a safe working environment for staff prior to the commencement of service.
  • Create and implement new client Master Schedules.
  • Provide client orientation and educate on the program goals, principles, and values.
  • Work with ILS management to recommend and revise processes related to intake and admissions.
  • Participate in presentations to educate community groups and partner agencies on ILS’s programs and eligibility.
  • Build professional relationships with collaborative partners, community support agencies and hospital discharge planners to ensure coordination of services.
Financialamp; Statistical Management
  • Assists with the collection of programs statistics.
  • Effectively logs and tracks all department statistics.
  • Completes program reports and statistics as required.
  • Works with the Database Administrator and Manager to ensure database accuracy.
Strategic Planning and Development
  • Participate in planning and communication of ILS’s missions, values and goals.
  • Monitor and report Program Targets/ work with Database Administrator to report on Admissions and Discharges for statistics.
Health and Safety
  • Adhere to the Agency’s Health and Safety Policies and Procedures and ensure a safe andhealthy workplace for all employees.
Required Qualifications and Skills
  1. Graduate of a post-secondary program in health care, social work and or a combination of related work experience required.
  2. Experience in intake and waitlist management related to community services or other related fields.
  3. Experience with interRAI assessment tools.
  4. Experience with the development of care plans with the focus of promoting the client’s independence.
  5. Competent with Microsoft Office Suite, Windows, Excel, and other current technologies.
  6. Experience working with databases, including Health Gateway Partner HPG.
  7. Excellent organizational, problem-solving, attention to details, analytical and strong research skills.
  8. Strong customer service focus and demonstrated knowledge and sensitivity to the needs of adults with disabilities and seniors.
  9. Effective communication skills, (verbal, written, active listening and presentation skills).
  10. Knowledge of community resources, and legislation related to community health services.
  11. Maintain reliable transportation and possess a valid Ontario Driver’s License.
  12. Maintain the confidentiality of informationamp; respect privacy laws.
Please Note that due to the changing nature of the ILS #39;s role and relationship with funders, the duties and responsibilities of this position may change from time to time necessitating the delegation of other related duties within the agency.Other ConsiderationsILS is hiring one 12-month Full-Time contract position with the possibility of extension. Scheduled time in the Barrie office will be required, as well as some travel within Simcoe County.
