Job Description

Reporting to the Regional Director, Central East Stroke Network (CESN), the Regional Community and Long-Term Care Coordinator (CLTCC) is a dedicated regional resource for CESN (Simcoe, Muskoka, York, Durham, Haliburton, Kawartha, Pine Ridge).Based out of the Enhanced District Stroke Centre at Royal Victoria Regional Health Centre, the CLTCC will support the integration of services for stroke survivors by developing and implementing a plan for uptake of stroke best practices amongst care providers and organizations involved in the provision of community and long-term care (LTC) services across the region. This is accomplished in collaboration with other members of the regional stroke team (Regional Director, Regional Education Coordinator, Regional Rehabilitation Coordinator), the District Stroke Centres, and stakeholders across the continuum of stroke care. This will include collaboration with health care, community service, and LTC providers, Home and Community Care Support Services, persons with stroke and their caregivers across the region, and other regional and provincial stakeholders as required.Building upon past work (i.e. needs assessments, environmental scans, surveys, regional stroke projects and workplans, provincial integrated workplans, provincial CLTCC initiatives) and opportunities for ongoing work (i.e. implementation of Canadian Stroke Best Practice Recommendations), the CLTCC will identify and build upon opportunities for system change, partnerships and consultation that will enhance access to stroke care expertise, best practices, effective transitions, and community reintegration supports in community and LTC settings.Communication and collaboration with many health care and community service partners is integral to the position at a local, regional and provincial level.Education:

  • University Degree in a health-related discipline; Master’s degree preferred in any health-related program such as Health Administration, Rehabilitation, or similar program.
  • Member in good standing of a college for a relevant regulated health profession under the Regulated Health Professions Act
Experience:
  • Minimum five years relevant experience as a regulated health professional in stroke care including two years of exposure to LTC and /or community care settings
  • Experience in a leadership role with the ability to work effectively with Health Care Administrators and Community Agencies
  • Experience with program development and change management including working with culturally diverse populations
Competencies:
  • Knowledge of the Ontario stroke system.
  • Knowledge of the complexities of LTC and its culture
  • Demonstrated knowledge of stroke best practices and the continuum of stroke care
  • Demonstrated project management, leadership and team building skills
  • Demonstrated ability to facilitate and manage system change/improvement
  • Demonstrated ability to foster an environment which promotes collaborative decision making
  • Demonstrated facilitation and networking skills with ability to function as a change agent and build capacity in the health care community
  • Critical thinking ability
  • Strong consultative skills combined with clinical, organizational, and community development knowledge
  • Appropriate understanding, sensitivity, flexibility and ability to work with multiple corporate cultures and culturally diverse populations
  • Demonstrated ability to work effectively independently (high level of self- direction and initiative) and as part of a team
  • Knowledge of research and evaluation methodologies
  • Ability to locate and critically appraise relevant professional literature
  • Ability to transfer knowledge and translate evidence into practice
  • Excellent verbal and written communication skills. Experience in delivering educational presentations with demonstrated effective presentation abilities
  • Ability to utilize and interpret data to monitor and inform stroke system priorities
  • Ability to share data and insights with organizations.
  • Demonstrated planning and organization and time management skills
  • Proven flexibility and ability to adapt positively to change
  • Demonstrated excellent interpersonal skills, including conflict resolution skills
Other:
  • Computer literacy is essential, with the ability to use the internet, Microsoft Word, Excel, PowerPoint, Visio, Teams, Outlook, and other programs as needed.

  • Valid driver’s license/ willing to travel throughout the region and occasionally provincially or out of the province; access to own transportation
