Reporting to the Regional Director, Central East Stroke Network (CESN), the Regional Community and Long-Term Care Coordinator (CLTCC) is a dedicated regional resource for CESN (Simcoe, Muskoka, York, Durham, Haliburton, Kawartha, Pine Ridge).Based out of the Enhanced District Stroke Centre at Royal Victoria Regional Health Centre, the CLTCC will support the integration of services for stroke survivors by developing and implementing a plan for uptake of stroke best practices amongst care providers and organizations involved in the provision of community and long-term care (LTC) services across the region. This is accomplished in collaboration with other members of the regional stroke team (Regional Director, Regional Education Coordinator, Regional Rehabilitation Coordinator), the District Stroke Centres, and stakeholders across the continuum of stroke care. This will include collaboration with health care, community service, and LTC providers, Home and Community Care Support Services, persons with stroke and their caregivers across the region, and other regional and provincial stakeholders as required.Building upon past work (i.e. needs assessments, environmental scans, surveys, regional stroke projects and workplans, provincial integrated workplans, provincial CLTCC initiatives) and opportunities for ongoing work (i.e. implementation of Canadian Stroke Best Practice Recommendations), the CLTCC will identify and build upon opportunities for system change, partnerships and consultation that will enhance access to stroke care expertise, best practices, effective transitions, and community reintegration supports in community and LTC settings.Communication and collaboration with many health care and community service partners is integral to the position at a local, regional and provincial level.Education:
