Catulpa Community Support Services Inc.Job Posting Competition: Internal/External #402-2022-23Full-Time 35 hours/week, 1-year ContractPOSITION: Transitional Age Youth Case ManagerLOCATION: Barrie, with responsibility throughout Simcoe CountyPURPOSE OF POSITIONTo develop individualized transition plans for young adults transitioning from Child Welfare orComplex Special Needs to the adult system seeking developmental services which identifyresponsibilities of individuals, families and service providers as the transition of service occurs.Transition planning is based on the individual’s eligibility, goals and strengths, assessed needsand available resources.STATEMENT OF MAJOR RESPONSIBILITIES

  • To provide the Ministry of Children, Community and Social Services with individual plans that include recommendations for fiscal and annualized budgets based on the young adult’s needs
  • To facilitate development and implementation of the transition support plan with the youth’s team
  • To provide young adults, their families/guardian and support teams with better access to information about the transition process and the range of adult services and supports;
  • To support informed decision making in adult service planning
  • To enhance transition readiness of individuals and families/guardian
  • support strong linkages between children’s and adult services, school, home and community
  • To facilitate service continuity and successful transition to adult Developmental Services.
  • The Transition Plan is completed by the TAY Coordinator, identifying agreed upon strategies, actions, responsibilities, time frames and key issues/questions impacting planning. The TAY Coordinator is responsible to disseminate meeting minutes/documentation to all team members
  • To assist in service system analysis to identify and communicate gaps and needs to agencies and funders
  • To record and report client information necessary for effective case management and prepare transition planning document
  • To collect and report service delivery data as required
  • To adhere to all agency standards, policies and procedures
  • To regularly attend and participate in supervision and to engage in annual performance appraisal
  • To participate in staff training as recommended by the manager and demonstrate initiative in learning and developing new techniques of service delivery
  • To carry out such other duties as may be assigned by the Supervisor that are consistent with the incumbent’s position
QUALIFICATIONSEducation and Experience
  • Post-secondary degree at a university level in one of the social or behavioral disciplines with directly related experience (3 to 5 years) in working with children/youth and adults with multiple and complex areas of exceptionality (developmental disability, emotional/psychiatric needs, medically fragile, acquired brain injury) and their families
Knowledge and/or Skill Level
  • Knowledge of normal and abnormal development.
  • Knowledge of family systems theory and an understanding of the family development and functioning.
  • Ability to develop trusting, co-operative relationships with clients, families, service providers, and other stakeholders.
  • Excellent communication and negotiating skills and judgment and ability to manage communication.
  • Computer/keyboarding skills are required.
  • Knowledge of community resources for youth and adults with special needs and relevant legislation.
  • Ability to support self-advocacy and empowerment for families.
  • Sensitivity, understanding and ability to work with a variety of value systems, beliefs and cultures.
  • Working knowledge of word processing and data entry
  • Capacity to work independently
  • Aware of Health & Safety Rules & Regulations.
  • A working knowledge of French Language is beneficial.
