Job Description

Position Summary:PRIMARY PURPOSEThe Clinical Manager – Wound Care Program will liaise with primary care physicians and contracted service providers to identify issues of concern, collaboratively develop strategies to work towards improvements, and provide recommendations for policies, procedures, and approach to wound care. These improvements are expected to lead to improved outcomes for patients. The Clinical Manager will utilize a patient centered approach to properly assess needs of patients within the Wound Care Program setting, including providing leadership and oversight to the Wound Care Team.Core Duties - Responsibilities:KEY RESPONSIBILITY AREAS

  • Assesses patients with complex wounds
  • Supports an interdisciplinary team with best practice treatment plans to improve wound outcomes for the target population
  • Provides leadership and oversees the wound care team of Care Coordinators and Patient Care Assistants
  • Incorporates current and future technology to support and further develop the Home & Community Care Support Services Hamilton Niagara Haldimand Brant (HCCSS HNHB) Wound Care Program
  • Provides comprehensive wound care services to patients as per scope of practice guidelines
  • Plans, develops and delivers programs and/or services related to the Wound Care Program
  • Initiates, benchmarks, evaluates, recommends, and implements best practices in the delivery of services
  • Supports an organizational culture that fosters excellence in the provision of care and multi- disciplinary teamwork
  • Collaborates with and provides expertise and guidance to Care Coordinators, Medical Supplies & Equipment Team, Nursing Care Centre and Wound Care Leads
  • Develops, implements and monitors for improvement policies and procedures
  • Participates in continuing education and training initiatives at HCCSS HNHB, including mentoring team members
  • Interacts with leaders to promote consistency and best practices
  • Participates in performance measurement and utilization management
  • Promotes communication to develop a collaborative multi-disciplinary care team
  • Promotes a continuous learning environment
  • Anticipates changing priorities and recommends reallocation of resources as required to meet overall needs
  • Monitors, tracks and prepares reports about the Wound Care Program
  • Leads and/or participates in and demonstrates an understanding of quality, risk and client safety principles and practices
  • Follows all safe work practices and procedures and immediately communicates any activity or action which may constitute a risk to quality, and client safety
Patient Safety
  • Promotes patient safety in alignment with the Vision, Mission, Values and Strategic Directions of the HCCSS
  • Works within the basic principles of patient safety by doing the right thing for the right patient, using the right method at the right time
  • Adheres to HCCSS patient safety policies and procedures
Qualifications:POSITION REQUIREMENTSEducational Qualifications
  • Certificate of Registration in the Extended Class from the College of Nurses of Ontario (CNO) and eligible to practice as a RN(EC) or Registered Nurse (Certificate of Registration in the General Class from CNO and eligible to practice as an RN) with a Master’s degree in nursing and experience as a Clinical Nurse Specialist.
  • Preferred 3 to 5 years’ experience in home or community care environment. Professional experience in health care should include: linkages with community health agencies and other health ;disciplines, needs assessment, and case management and/or discharge planning
  • Completion of a recognized wound care program and/or course preferred.
  • Wound, Ostomy, Continence (WOCC(C)) certification from the Canadian Nurses Association preferred.
  • Current Basic Life Support (BLS) in accordance with the Heart and Stroke Foundation of Canada or CPR Level C & AED.

  • Knowledge of HCCSS mission, vision, and strategic directions
  • Knowledge of best practices related to service delivery
  • Significant understanding of HCCSS relationship with broader patient service community
  • Ability to research, analyze, and evaluate best practices program development and implementation and in service delivery

  • Must have own transportation

  • Excellent training and presentation skills
  • Proven ability to work with stakeholders to support collaborative continuous improvement initiatives
  • Proficient with Microsoft Office Word, Excel and PowerPoint
  • Demonstrated ability to work with teams and individuals to achieve success
  • Establishes and maintains collaborative and effective working relationships with HCCSS employees, physicians, and with peers throughout HCCSS
  • Provides leadership in meeting patients’ primary care needs and achieving pilot project goals
Other:
  • Drivers licence and access to/use of personal vehicle for travel throughout HNHB HCCSS area
