Job Description

Status: Full-time, Temporary (Expected Contract End Date: March 31st, 2026)Vacancies: OneFTE: 1.0Schedule: Monday - Friday, 8:30 a.m. - 4:30 p.m. Location: Calgary ZoneBenefits: Yes Salary range: $42,000 - $51,000/yrThe Calgary West Central Primary Care Network (CWC PCN) is a non-profit organization committed to a healthier future. We work alongside more than 490 family doctors to provide support for continuous health care. We are seeking a Patient Care Coordinator (PCC) to join our Patient’s Medical Home (PMH) team. In this role, the PCC supports physicians and PMH team members by coordinating patient care to improve overall patient outcomes. This role is primarily a work from home position with some occasional in-person work at our Main Office or physician clinics in the Calgary area. Therefore, the successful candidate must be located within Calgary or the surrounding area (e.g., Cochrane, Airdrie, Chestermere, etc.).This is a full-time, temporary contract (maternity leave cover). The contract is expected to end on March 31st, 2026. ResponsibilitiesPanel management and EMR standardization:

  • Support routine panel maintenance and patient validation processes for ongoing panel management, such as confirming primary medical doctor, demographics, and validating patient information while performing outreach to patients.
  • Identify missing vitals, lab results, or screening, referrals to external organizations, etc.
  • Ensure results of recent tests and procedures are available to providers (i.e., information is available on Alberta Netcare).
  • Create and maintain accurate notes about projects such as meeting minutes and ad hoc patient list requests.
Coordination of patient care:
  • Reach out to patients identified in projects (e.g., telephone calls, email, text).
  • Support clinicians with scheduling, using Electronic Medical Records (EMRs) and digital health platforms.
  • Coordinate patient care with the appropriate clinicians, including referral processing and ensuring patients are informed about any appointment information required.
  • Provide further instruction or information as required to patients prior to the visit.
  • Coordinate patient care by referral to the medical neighbourhood (i.e., patients identified in a PMH project that require care outside of the Patient’s Medical Home).
RequirementsEducation: A Medical Office Assistant (MOA) certificate or diploma, or a post-secondary diploma or certificate in a related area such as Licensed Practical Nurse, Medical Secretary, or Unit Clerk, is required. Equivalencies of education may also be considered. Experience: A minimum of two years of experience in the following is required:
  • Strong administrative experience working within an interprofessional care team model.
  • Knowledge and experience with medical records management.
  • Ability to maintain filing systems and basic databases.
A minimum of two years of experience in the following is an asset:
  • Previous experience in handling confidential or sensitive information.
  • Healthcare experience, preferably in a primary care setting.
Functional knowledge:
  • Knowledge of medical terminology and medical records management.
  • Knowledge of applicable data privacy laws.
  • Knowledge of and interest in primary health care and health promotion.
Technical: Microsoft Office (Word, Excel, Outlook, PowerPoint, Teams). Virtual patient care platforms. Electronic Medical Records (EMR) systems.Background check: A clear Criminal Background check is required to start.
