Job Description

Your Opportunity:Reporting to the Rural South Area Manager and as part of the Rural South Home Care Management team, this role supports an integrated interdisciplinary team of mobile professionals and support staff in the community. There is a strong focus on client centered care, the development and monitoring of staff and material resources. Supporting staff is fundamental to this position. Strong communication skills facilitate the internal function of this role and promote as well as maintain external partnerships essential in the delivery of care. The Care Manager acts as a change agent facilitating the evolving nature of the Integrated Home Care Program. This position assists with leading the Rural South Team which is compromised of Black Diamond, Okotoks, High River and Nanton, but is primarily responsible for the Claresholm & Vulcan communities.Description:Leadership and Planning: Manages the day to day operations of professional staff at the site level, brings forward concerns and recommends solutions to the Area Manager. Identifies staffing needs required to effectively and efficiently operate. Monitors operational issues, acts as a role model and change agent to ensure the implementation and success of regional and provincial strategic directions. Service Delivery: Fosters client centered care, recognizing unique client situations and complexity of care delivery in the community. Makes operational decisions within the assigned area of responsibility based on critical review of clinical information, workload data and fiscal resources. Resource Management: Participates in the entire recruitment process: identifying need, interviewing, hiring, and providing orientation to newly hired staff. Encourages and supports staff to identify learning needs; promotes continued learning opportunities for professional growth. Fosters an atmosphere conducive to meeting staff development and student learning experiences. Monitors and comments on staff performance, provides immediate, constructive feedback to facilitate ongoing quality performance. Completes staff performance reviews. Coordination and Communication: Fosters internal/external information exchange related to areas of responsibility with various stakeholders. Facilitates/leads regular team meetings providing staff with current guidelines and process information impacting clinical/case management practice. Required Qualifications:Bachelor Degree in Health-care field, Business or Administration required; however, consideration will also be given to those actively enrolled in a Bachelor’s degree program; Master in Healthcare Administration, Business Administration or Clinical specialty preferred. Minimum 2-5 years previous related experience including management at an entry level. Demonstrated decision-making abilities and strong interpersonal, conflict resolution skills required. Leadership skills essential with knowledge of change management; conflict resolution frameworks and coaching models as asset. Valid drivers license and access to a reliable motor vehicle for day to day travel.Additional Required Qualifications:As Required.Preferred Qualifications:Knowledge of regional administration, personal computing systems and budget development would be an asset.
