Job Description

Your Opportunity:Come and join the passionate and diverse team at the Willow Creek Continuing Care Centre (Long Term Care) in Claresholm, Alberta. Nestled in the Porcupine Hills, Claresholm is close to many outdoor activities including camping, hiking, and fishing. Under the supervision of the Patient Care Manager of Long Term Care, the Licensed Practical Nurse (LPN) works as a member of a health care team, collaborating on the delivery of care to meet the needs of patients/clients. The LPN assesses, plans, implements and evaluates care of patients/clients and work in consultation with RN and colleagues to meet the needs of more complex patients/clients. This position will: direct and provide resident care, supervise and schedule staff as necessary, collaborate with families and residents and the interdisciplinary team. As one of the leaders of the front-line team, you will ensure the care needs of residents are being met in a comprehensive and timely manner. The LPN is responsible and accountable to the public for his/her practice, according to the Standards defined by the College of Licensed Practical Nurses of Alberta (CLPNA) Standards of Practice (2003). Working in this position you can qualify for the Canadian Student Loan Forgiveness for Nurses. If you are a nurse or nurse practitioner and you qualify, you could receive up to $20,000 in Canada Student Loan forgiveness over a maximum of five years ($4,000 per year). You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).Additional Required Qualifications:Ability to work as a member of a team and to request and accept direction as required. Demonstrates critical thinking skills, with in scope of practice, including clinical judgment, problem solving skills and strong decision making ability. Dementia experience is required. Strong written and verbal communication skills are required as is a commitment to integrating Patient and Family Centered Care principles into your practice. The ability to adapt quickly to changing and fluctuating workloads will be vital. A commitment to promoting a positive work environment and to professional development is valued.Preferred Qualifications:Previous LTC experience is an asset. Geriatric experience. Supportive living and/or continuing care experience is an asset. Supportive pathways an asset. Palliative experience is an asset.
