Your Opportunity:We are actively seeking a Rehabilitation Practitioner I to work within the Claresholm Centre for Mental Health & Addictions, a 108 bed facility in the town of Claresholm. Claresholm is a vibrant rural community that is located approximately one and a half hours drive from Calgary and one hour drive from Lethbridge. This position reports to the Site Managers and is instrumental in designing and implementing goal-orientated rehabilitation services for mental health clients on assigned caseload. Prepares detailed assessments on work skills and behaviors at specific intervals and documents findings on the Treatment Team notes in the clients clinical record. Assess clients and makes treatment plan recommendations using approved tools. Provides individual vocational counseling by examining occupational goals, education and barriers while working with team to implement treatment care plans. Will schedule re-assessments and updates plans regularly while attending all required meetings and participating in Voc Rehab service development by sharing ideas with the Voc Rehab team members.Description:As a Rehabilitation Practitioner I, you will work with the patient care team by delivering individual and group programs.
